结直肠癌患者ZO-1等生化指标水平及其临床意义

摘　　要：目的探讨结直肠癌患者外周血中紧密连接蛋-1(ZO-1)、咬合蛋白(Occludin)、活化白细胞细胞粘附分子(ALCAM)、血小板内皮细胞粘附分子-1(PECAM-1)的检测水平及其临床意义。方法选择50例结直肠癌患者为研究对象,选择肠息肉患者、健康者50名作为对照。采用ELISA法检测各组对象血清中ZO-1、Occludin、ALCAM、PECAM-1水平。结果结直肠癌组血清ALCAM、PECAM-1水平高于肠息肉组和健康组,差异有统计学意义(P<0.05);结直肠癌组血清ZO-1、Occludin水平低于肠息肉组和健康组,差异有统计学意义(P<0.05)。Ⅲ~Ⅳ期、伴淋巴结浸润、伴肝转移患者血清ALCAM、PECAM-1水平高于Ⅰ~Ⅱ期、不伴淋巴结浸润、不伴肝转移患者,差异有统计学意义(P<0.05);Ⅲ~Ⅳ期、伴淋巴结浸润、伴肝转移患者血清ZO-1、Occludin水平低于Ⅰ~Ⅱ期、不伴淋巴结浸润、不伴肝转移患者,差异有统计学意义(P<0.05)。ZO-1、Occludin水平与临床分期、伴肝转移、伴淋巴结浸润呈负相关;ALCAM、PECAM-1水平与临床分期、伴肝转移、伴淋巴结浸润呈正相关。结论 ZO-1、Occludin、ALCAM、PECAM-1在结直肠癌患者外周血血清中呈现异常表达,ZO-1、Occludin、ALCAM、PECAM-1水平与结直肠癌的发病及进展密切相关。结直肠癌患者存在肠黏膜屏障功能损伤、血管内皮功能的紊乱,且肠黏膜屏障功能损伤、血管内皮功能紊乱与结直肠癌的临床病理有关。Objective To study the peripheral blood ZO-1,Occludin,ALCAM,PECAM-1 levels and their significance in patients with colorectal cancer. Methods 50 patients with colorectal cancer were selected as the research object.50 patients with acute pancreatitis and 50 patients with healthy physical examination were selected as the control groups.The ZO-1,Occludin,ALCAM,PECAM-1 levels of the 3 groups were detected by ELISA method. Results In colorectal cancer group ALCAM,PECAM-1 levels were higher than that of pancreatitis group and the healthy group,and the difference was statistically significant（ P 〈0.05）;In the colorectal cancer group ZO-1,Occludin level was lower than the pancreatitis group and the healthy group,and the difference was statistically significant（ P 〈0.05）.Of patients with stage Ⅲ~Ⅳ colorectal cancer,ALCAM,PECAM-1 levels were higher than that of patients with stage Ⅰ~Ⅱ colorectal cancer,and the difference was statistically significant（ P 〈0.05）;Patients with stage Ⅲ~Ⅳ colorectal cancer ZO-1,Occludin levels were lower than that of stage Ⅰ~Ⅱ patients with colorectal cancer,and the difference was statistically significant（ P 〈0.05）.Of colorectal cancer patients with lymph node invasion,ALCAM,PECAM-1 levels were higher than that of patients without lymph node invasion,and the difference was statistically significant（ P 〈0.05）;Of colorectal cancer patients with lymph node invasion,ZO-1,Occludin levels were lower than that of patients without lymph node invasion,and the difference was statistically significant（ P 〈0.05）.In colorectal cancer patients with liver metastasis,ALCAM,PECAM-1 levels were higher than that of patients without liver metastasis,and the difference was statistically significant（ P 〈0.05）;In colorectal cancer patients with liver metastasis,ZO-1,Occludin levels were lower than that of patients without liver metastasis,and the difference was statistically significant（ P 〈0.05）.The lever of ZO-1 and Occludin showed a negative corre
