摘 要:目的探讨食管癌术后患者化疗后血清蛋白指纹图谱变化及其意义。方法收集18例食管癌术后1周患者血清,15例化疗后患者血清(同期手术后),20例健康志愿者血清,采用WCX磁珠对血清进行分析前处理,MALDITOF MS技术进行血清蛋白谱检测,应用人工神经网络(SNN)算法建立食管癌术后、化疗后的诊断模型,将53例样本随机划分,80%作为训练组,20%作为盲法组,验证诊断模型的特异度和灵敏度。结果食管癌术后组、化疗后组患者血清蛋白与健康志愿者血清蛋白,经过对比数据分析,筛选出差异最显著的2个蛋白质荷比峰建立诊断模型,其中食管癌术后组(m/z 676.61)、化疗后组(m/z 676.46),上述诊断模型的特异度100%和灵敏度100%。比较术后组与化疗后组血清蛋白建立诊断模型,化疗后组(m/z 3951.77,2661.11,823.61,824.08,5907.51,6670.7,4266.81,810.47,4965.26,1519.3,849.63,1466.09,1617.58,1467.16,759.8),分组正确率为100%。结论 WCX磁珠和MALDI-TOF MS技术能够检测蛋白指纹图谱,建立食管癌手术后、化疗后的诊断模型,模型具有较高的灵敏度和特异度。筛选到诊断和判断化疗有意义的蛋白峰。化疗导致了血清蛋白变化,其发生机制不清楚,有待于进一步研究。Objective To confirm serum peptidome pattern of esophageal cancer patients treated with surgery after and chemotherapy,and its clinical value. Methods Serum samples were collected from33 patients( 18 samples of EC operated,15 samples of chemotherapy) 20 samples of Healthy volunteers were selected. Spectral data generated with weak cationic-exchanger magnetic beads( WCX-MB) and MALDI-TOF MS by a GA,were used to construct diagnostic models and system training as potential biomarkers. 53 samples were randomly divided into training set(80%) and blinding test sets(20%) to examine the specificity and sensitivity of this diagnostic model. Results A pattern consisting of 2 protein peaks,separated EC operated( m/z676. 61),and chemotherapy( m/z 676. 46) from the healthy individuals with a sensi-tivity of 100. 0% and a specificity of100. 0%. EC operated chemotherapy( m/z3951. 77,2661. 11,823. 61,824. 08,5907. 51,6670. 7,4266. 81,810. 47,4965. 26,1519. 3,849. 63,1466. 09,1617. 58,1467. 16,759. 8) from EC operated. Recognition Capability 100%. Conclusion A combined application of magnetic beads with MALDI-TOF MB technique are suitable for identification of potential serum biomarkers for ECoperated and chemotherapy and it is a promising way to establish a diagnostic pattern. Serum proteinic markers with potential diagnostic value and prognostic efficacy value may be found in EC operated and chemotherapy. The mechanism is not clear,needs further research.
