摘  要:目的:采用口腔全景X线片,对上颌牙槽骨高度的区域性差异及其特点进行测量分析,为牙弓牙槽弓新分区及其临床应用价值提供支持。方法:随机选取200例正常成年上颌骨X线全景片,根据笔者团队提出的牙弓牙槽弓分区,分别对弓顶区、弓肩区和弓体区各牙位牙槽骨高度进行测量,弓顶区测量牙槽嵴顶至骨性鼻底的高度,弓肩区测量牙槽嵴顶至骨性鼻底与骨性上颌窦底高度,弓体区测量牙槽嵴顶至骨性上颌窦底的高度(h),统计各牙位牙槽骨平均高度与各区域牙槽骨平均高度最低牙位,同时以h≤6mm,6mm<h≤8mm,8mm<h≤10mm,10mm<h≤12m,12mm<h≤14mm,h>14mm为参考区间,分别统计不同牙位牙槽骨高度在各高度区间的百分比。结果:正常成年人弓顶区中切牙牙槽骨高度最低,平均为24.8mm,弓肩区第二前磨牙牙槽骨高度最低,平均为16.8mm,其牙槽骨高度在不同区间百分比分别为1.75%,4.25%,9.25%,12.75%,14%与58%,弓体区第一磨牙牙槽骨高度最低,平均为12.7mm,其牙槽骨高度在不同区间的百分比分别为9.5%,20.25%,14%,12%,11.75%与32.5%。结论:上颌牙槽骨高度区域性差异明显,新牙弓牙槽弓分区既能较好地体现这种差异,也能很方便地将这种差异转化为临床实际应用。Objective To evaluate the alveolar height difference and the characteristics of different maxillary area by measurement of panoramic radiograph. Methods: 200 panoramic radiographs of normal adults were randomly selected. The alveolar height of each tooth of different area according to the new area division was measured. The height was concretely measured from alveolar top to basis nasi at the arch apex area, from alveolar top to basis nasi and sinus floor at the arch shoulder area and from alveolar top to sinus floor at the arch body area. The lowest alveolar height of each area and the height percentage of each tooth in different height interval was analyzed. The intervals were divided as h≤6 mm, 6 mm〈h≤8 mm, 8 mm〈h≤10 mm, 10 mm〈h≤12 mm, 12 mm〈h≤14 mm and h 〉14 mm. Results: The lowest alveolar height of arch apex area was at central incisor site, and the average height was 24.8 mm. The lowest alveolar height of arch shoulder area was at second premolar site and the average height was 16.8 mm. The distribution percentage of alveolar height of this tooth in different height interval were 1.75%, 4.25%, 9.25%, 12.75%, 14% and 58%. The lowest alveolar height of arch body area was at first molar site and the average height was 12.7 mm. The distribution percentage of alveolar height of this tooth in different height interval were 9.5%, 20.25%, 14%, 12%, 11.75% and 32.5%. Conclusion: There was obvious area characteristics of alveolar height difference in maxillary jaw. The new dental and alveolar arch division could reflect this difference and translate the difference into clinical application in dental implantation.
