摘 要:目的探讨蛛网膜下腔出血患者责任动脉瘤CT血管造影(CTA)特征。方法选取2014年1月至2018年3月间我院诊治的颅内多发动脉瘤患者30例为研究对象,共39个动脉瘤,经检测确认破裂动脉瘤30个,未破裂动脉瘤9个,比较两者CTA形态学特征。结果破裂动脉瘤瘤颈宽、高度、长径、AR、BN和SR值均明显大于未破裂动脉瘤,差异有统计学意义(P<0.05)。ROC曲线分析可见,AR、SR曲线下面积最高,分别为0.793和0.837,对应的最佳临界值则分别为1.38和1.84。多因素Logistic回归分析得出AR和SR与动脉瘤破裂具有独立相关性。破裂动脉瘤中,形态不规则的比例为66.7%,明显高于未破裂动脉瘤的22.2%,差异有统计学意义(P<0.01)。结论 CTA下责任动脉瘤特征表现为AR和SR数值较大(临界值分别为1.38和1.84),瘤体形态不规则。Objective To investigate the characteristics of CT angiography(CTA)in patients with subarachnoid hemorrhage.Methods 30 cases of intracranial multiple aneurysms were selected from Jan 2014 to Mar 2018 in our hospital.There were 39 aneurysms,30 ruptured aneurysms and 9 unruptured aneurysms were identified.The morphological features of CTA were compared.Results the diameter width,height,long diameter,AR,BN and SR values of ruptured aneurysms were significantly higher than those of unruptured aneurysms(P〈0.05).ROC curve analysis showed that the area under the AR and SR curves was the highest,0.793 and 0.837 respectively,and the corresponding optimal threshold values were 1.38 and 1.84 respectively.Multivariate Logistic regression analysis showed that AR and SR were independently associated with aneurysm rupture.The ratio of irregular shape in ruptured aneurysms was 66.7%,which was significantly higher than that in 22.2%of unruptured aneurysms(P〈0.01).Conclusion The characteristics of AR aneurysms under CTA are larger than those of SR(the critical value is 1.38 and1.84 respectively).The more irregular the tumor shape is.
