摘　　要：目的探讨狗咬伤致上唇缺损后一期创口处理原则,及采用改良Abbe-Estlander瓣针对不同位置的上唇缺损进行个性化修复的效果。方法回顾性研究2006年1月至2016年7月,中国医科大学附属口腔医院口腔颌面外科治疗因狗咬伤致上唇缺损患者53例。通过查阅病例资料、电话随访、门诊复查、术前术后照片对比等手段获取所需信息。结果术后平均随访时间9.4个月,无因感染狂犬病毒而出现死亡病例;98.1%的患者术后创口愈合良好;术后所有皮瓣均成活,且无明显并发症;患者对于唇外形的整体满意度达96.2%;所有患者术后唇完整性及唇功能良好。结论狗咬伤致上唇缺损可进行一期修复重建,而采用改良Abbe-Estlander瓣术式修复上唇缺损具有较好的修复效果,且术后唇形态和功能恢复较好。Objective To explore the principle of primary wound management after upper lip defects caused by dog bites and evaluate the efficacy of upper lip reconstruction by modified Abbe-Estlander flap. Methods From January 2006 to July 2016, 53 patients with upper lip defects caused by dog bites in the department of Oral and Maxillofacial Surgery of the Stomatology Hospital affiliated to China Medical University were retrospectively reviewed. The required information was obtained by medical records, telephone follow-up, outpatient review, and pre-and post-operative comparison of photos. Results The average follow-up duration was 9.4 months. No deaths occurred due to rabies virus infection. About 98.1% of patients healed well after operation. All flaps survived and no obvious complications occurred. Patients＇ overall satisfaction with the appearance of the lip was 96.2%. All patients have good integrity and function of the lip.Conclusion Defects of the upper lip caused by dog bites can be primarily repaired and reconstructed. The modified Abbe-Estlander flap is a very good choice for repairing upper lip defects after dog bites. The recovery of appearance and function of the lip was better after surgery.
