奥曲肽联合加贝酯对重症急性胰腺炎患者相关因子指标的影响

摘　　要：目的:探讨奥曲肽联合加贝酯对重症急性胰腺炎(severe acute pancreatitis,SAP)患者血清协同刺激分子B7家族3(B7-H3)、降钙素原(procalcitonin,PCT)、可溶性细胞间黏附分子-1(soluble Intercellular AdhesionMolecule-1,sICAM-1)、核转录因子κB(nuclear transcription factor-κB,NF-κB)、可溶性髓样细胞表达的激发受体-1(soluble triggering receptor expressed on myeloid cells-1,sTREM-1)、B细胞激活因子(B cell activating factor,BAFF)、淀粉样蛋白A(serum amyloid A,SAA)、中性粒细胞明胶酶相关载脂蛋白(neutrophil gelatinase-associated lipocalin,NGAL)、人生长激素释放肽(ghrelin)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)、超氧化物歧化酶(superoxide dismutase,SOD)、丙二醛(malondialdehyde,MDA)水平的影响。方法:选择2014年1月~2017年12月我院收治的92例SAP患者,随机分为对照组和实验组,各46例。对照组患者给予奥曲肽治疗,实验组患者给予奥曲肽联合加贝酯治疗。比较两组患者治疗前后血清B7-H3、PCT、sICAM-1、NF-κB、sTREM-1、BAFF、SAA、NGAL、ghrelin、hs-CRP、MDA及SOD水平。结果:两组治疗前的血清B7-H3、PCT、sICAM-1、NF-κB、sTREM-1、BAFF、SAA、NGAL、ghrelin、hs-CRP、MDA及SOD水平比较,无显著性差异(P>0.05);两组治疗后的血清B7-H3、PCT、sICAM-1、NF-κB、sTREM-1、BAFF、SAA、NGAL、ghrelin、hs-CRP及MDA水平均比治疗前低,血清SOD水平均比治疗前高,且实验组治疗后血清B7-H3、PCT、sICAM-1、NF-κB、sTREM-1、BAFF、SAA、NGAL、ghrelin、hs-CRP、MDA及SOD水平变化均优于对照组,差异均具有统计学意义(P<0.05)。结论:奥曲肽联合加贝酯能够明显降低SAP患者血清B7-H3、PCT、sICAM-1、NF-κB、sTREM-1、BAFF、SAA、NGAL、ghrelin、hs-CRP、MDA水平,提高血清SOD水平,值得在临床上推广应用。Objective：To study the effects of octreotide combined with gabexate on related cytokines in patients with severe acute pancreatitis（SAP）.Methods：A total of 92 patients with SAP from January 2014 to December 2017 in our hospital were enrolled in this study.According to random number table method,the patients were divided into the control group and the treatment group,with 46 patients in each group.The control group were treated with octreotide,while the treatment group were treated with octreotide combined with gabexate.Serum B7-H3,PCT,sICAM-1,NF-κB,sTREM-1,BAFF,SAA,NGAL,ghrelin,hs-CRP,MDA and SOD of the two groups before and after treatment were compared.Results：The serum B7-H3,PCT,sICAM-1,NF-κB,sTREM-1,BAFF,SAA,NGAL,ghrelin,hs-CRP,MDA and SOD of the two groups before treatment had no no significantly differences（P〉0.05）.The serum B7-H3,PCT,sICAM-1,NF-κB,sTREM-1,BAFF,SAA,NGAL,ghrelin,hs-CRP,MDA of the two groups after treatment were significantly lower than before treatment（P〈0.05）,the serum SOD of the two groups after treatment were significantly better than before treatment（P〈0.05）,and those of the treatment group were significantly better than the control group after treatment（P〈0.05）.Conclusion：Octreotide combined with gabexate for patients with SAP can reduce the serum B7-H3,PCT,sICAM-1,NF-κB,sTREM-1,BAFF,SAA,NGAL,ghrelin,hs-CRP,MDA levels,and improve serum SOD levels,so it was worthy of clinical application.
