摘  要:目的:探讨丁苯酞联合阿司匹林及阿托伐他汀对急性脑梗死患者脑血流状态及斑块稳定性的影响。方法:116例急性脑梗死患者,依据治疗方法分为对照组和观察组,每组各58例。对照组患者给予阿托伐他汀治疗,观察组患者在对照组的基础上给予丁苯酞注射液,之后对两组患者血流状态及斑块稳定性水平进行检测。结果:治疗后,两组患者血流峰速度(Vp)及平均血流速度(Vm)水平显著升高(P<0.05),而双侧大脑中动脉血流差值(DVp及DVm)水平显著降低(P<0.05),观察组患者治疗后,Vp及Vm水平分别为(73.25±0.89)cm/s和(36.79±0.33)cm/s,明显高于对照组治疗后(P<0.05),DVp及DVm水平分别为(17.56±0.67)cm/s和(7.86±0.85)cm/s,明显低于对照组治疗后(P<0.05);治疗后,两组患者颈动脉内-中膜厚度(IMT)、斑块面积、斑块数及低回声斑块例数均明显降低(P<0.05),观察组治疗后,IMT、斑块面积及斑块数分别为(1.08±0.07)mm、(0.55±0.05)mm2及(2.28±0.04)个,低回声斑块患者8例,均显著低于对照组(P<0.05);治疗后,TC、TG及LDL-C均明显降低(P<0.05),而HDL-C水平却明显升高(P<0.05),其中,观察组患者TC、TG及LDL-C水平在治疗后分别为(2.95±0.07)mmol/L、(1.59±0.08)mmol/L和(1.88±0.10)mmol/L,明显低于对照组(P<0.05),而HDL-C水平为(1.58±0.04)mmol/L,明显高于对照组(P<0.05)。结论:丁苯酞联合阿司匹林及阿托伐他汀能有效改善急性脑梗死患者脑血流状态,增加斑块稳定性,降低血脂水平,促进患者康复。Objective: To investigate the effect of butylphthalide combined with aspirin and atorvastatin on cerebral blood flow state and plaque stability in patients with acute cerebral infarction. Methods: 116 patients with acute cerebral infarction were retrospectively selected. All patients were divided into control group and observation group according to the treatment method,58 patients in each group. The control group was treated with atorvastatin,the observation group was given butylphthalide on the basis of the control group,and the blood flow state and plaque stability of the two groups were tested. Results: After treatment,the levels of Vp and Vm were significantly increased( P〈0. 05),while the levels of DVp and DVm were significantly decreased( P〈0. 05). After treatment,the Vp and Vm levels in the observation group were( 73. 25 ± 0. 89) cm/s and( 36. 79 ± 0. 33) cm/s,respectively,which were significantly higher than those in the control group( P〈0. 05),the DVp and DVm levels were( 17. 56 ± 0. 67) cm/s and( 7. 86 ± 0. 85) cm/s,which were significantly lower than those in the control group( P〈0. 05). After treatment,the levels IMT,plaque area,plaque number and the number of hypoechoic plaque in the two groups were significantly decreased( P〈0. 05). The levels of IMT,plaque area and plaque number in the observation group were( 1. 08 ± 0. 07) mm,( 0. 55 ± 0. 05) mm2 and( 2. 28 ± 0. 04),8 patients with hypoechoic plaque were significantly lower than those in the control group( P 0. 05). After treatment,TC,TG,and LDL-C all decreased significantly( P〈0. 05),while HDL-C levels increased significantly( P〈0. 05),and TC,TG and LDL-C levels in the observation group after treatment( 2. 95 ± 0. 07) mmol/L,( 1. 59 ± 0. 08) mmol/L and( 1. 88 ± 0. 10) mmol/L,respectively,which were significantly lower than those in the control group( P〈 0. 05),The level of HDL-C was( 1. 58 ± 0. 04) mmol/L,which was significantly highe
