根治性切除联合瘤床^(125)I放射性粒子植入治疗胰体尾癌疗效评价

摘　　要：目的研究胰体尾癌切除联合瘤床^(125)I粒子植入治疗胰体尾癌的临床疗效。方法胰体尾癌病人34例,按自愿原则分为两组A组19例,采取R0切除,B组15例,采取R0切除联合瘤床^(125)I粒子植入。术后均辅助化疗。比较两组病人的术后并发症、不良反应、生存期;KaplanMeier法计算中位生存期、绘制生存曲线,并用Log-rank检验。结果 A、B两组术后并发症发生率分别为26.3%(5/19)、26.6%(4/15),差异无统计学意义(P>0.05);术后化疗期间A、B两组恶心呕吐、骨髓抑制、肝功能损害、腹泻、疲劳不良反应发生率分别为10.5%、6.6%,21.1%、26.6%,10.5%、6.6%,15.7%、20.0%,26.3%、26.6%,比较差异无统计学意义(P>0.05);A组1、2、3年生存率分别为63.2%、36.8%和10.5%,B组分别为93.3%、73.3%和40.0%,组间生存率比较差异有统计学意义(P<0.05)。结论根治性切除术辅助^(125)I粒子瘤床植入治疗胰体尾癌有效、安全,能提高远期存活率。Objective To study the curative effect of pancreas carcinoma of the body and tail treated by radical resection of the body and tail of the pancreas combined with^125I particles implantation.Methods 34 patients with carcinoma of the body and tail of pancreas were selected as the research object. They were divided into A group（ radical resection）,B group（ radical resection combined with^125I particle implantation）. All patients received adjuvant chemotherapy postoperatively. The postoperative complications,adverse reactions and survival time were compared between the two groups. Kaplan-Meier method was used to calculate the median survival and the survival curve,and the Log-rank test was used.Results There was no significant difference between group A[26. 3%（5/19）]and group B[26. 6%（4/15）] of postoperative complications（ P〈0. 05）. During postoperative chemotherapy,the incidence of adverse reactions including nausea and vomiting,bone marrow suppression,liver function damage,diarrhea and fatigue in groups A（ 10. 5%,21. 1%,10. 5%,15. 7%,26. 3%） and B（ 6. 6%,26. 6%,6. 6%,20. 0%,26. 2%） were no significant difference（ P〈0. 05）;The survival rate of 1 year,2 year,3 year in group B（93. 3%,73. 3%,40. 0%） were significantly improved compared with those in group A（63. 2%,36. 8%,10. 5%）（ P〈0. 05）. Conclusion Radical resection combined with125 I particle implantation is effective and safe in the treatment of carcinoma of the body and tail of pancreas.
