摘 要:目的研究三七花总皂苷对大鼠脑缺血再灌注损伤模型保护作用,并探讨其作用机制。方法 SPF级Wistar大鼠120只,随机分为假手术组、模型组、阳性对照组(尼莫地平组20 mg/kg)、三七花总皂苷大、小剂量组(200、100 mg/kg);除假手术组外,其余各组采用改良线栓法,建立大鼠局灶性脑缺血再灌注损伤动物模型(MCAO),造模结束后,神经缺损评分(NDS)法评价行为学的改变、苏木素-伊红(HE)染色法评价脑组织病理形态的改变;酶联免疫吸附法(ELISA)检测肿瘤坏死因子(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-10(IL-10)、神经元特异性烯醇化酶(NSE)、B淋巴细胞瘤-2蛋白(Bcl-2)、Bcl-2相关的x蛋白(Bax)、半胱氨酸天冬氨酸蛋白酶-3(Caspase-3)含量水平,探讨三七花总皂苷对大鼠局灶性脑缺血再灌注损伤保护作用。结果与假手术组相比,模型组大鼠脑组织病理损伤严重,神经功能评分明显升高(P<0.05),TNF-α、IL-1β、NSE、Bax、Bcl-2、Caspase-3含量水平显著升高,IL-10降低,差异有统计学意义(P<0.05);与模型组相比,三七花总皂苷大剂量组能明显改善脑组织病理损伤程度,减缓神经功能损伤(P<0.05);三七花总皂苷大剂量组、尼莫地平片组能显著降低TNF-α、IL-1β、NSE、Bax、Caspase-3含量水平(P<0.05),提升IL-10、Bcl-2含量水平(P<0.05)。结论三七花总皂苷对局灶性脑缺血再灌注大鼠有一定的保护作用,其作用机制可能与降低TNF-α、IL-1β、NSE、Bax、Caspase-3含量水平、提高IL-10、Bcl-2水平、减轻炎症反应、抑制细胞凋亡有关。Objective To observe the protective effects of Panax notoginseng flower saponins(PNFS)on cerebral ischemia reperfusion injury in rats. Methods 120 SPF grade Wistar rats were randomly divided into sham operation group, model group, positive drug group(nimodipine, 20 mg/kg), PNFS high dose and low dose groups(200, 100 mg/kg). Models of middle cerebral artery occlusion(MCAO) were established in all the rats except those in sham operation group. The behavioral injury was detected by neurologic deficit scores(NDS); the cerebral histopathology was detected by HE staining; and the levels of neuron specific enolase(NSE) in serum, interleukin-10(IL-10), tumor necrosis factor-α(TNF-α), interleukin-1β( IL-1β), and the level of B-cell lymphoma-2(Bcl-2),Bcl-2 associated x protein(Bax),Cysteine aspartate protease(Caspase-3)in brain homogenate were detected by ELISA. Results As compared with sham operation group, the neurologic deficit scores were significantly higher in rats of model group(P〈0.05), with serious cerebral histopathology injuries; and the levels of NSE, TNF-α, IL-1β, Bax, Bcl-2 and Caspase-3 were significantly increased,IL-10 were significantly decreased(P〈0.05). As compared with the model group, the neurologic deficit symptoms were significantly improved in PNFS large dose group, cerebral histopathology injury was significantly decreased(P〈0.05). TNF-α, IL-1β, NSE, Bax and Caspase-3 levels were decreased(P〈0.05), and the levels of IL-10 and Bcl-2 were increased(P〈0.05) in PNFS large dose group and nimodiping group.Conclusion PNFS had the protective effects on MCAO rats by decreasing the level of TNF-α, IL-1β, NSE, Bax and Caspase-3, increasing the levels of Bcl-2 and IL-10, inhibition of apoptosis and hence attenuating the inflammation injury.
