白内障超声乳化摘除并植入后倾角人工晶体术后对后囊混浊的影响

摘　　要：目的探讨白内障超声乳化摘除并植入后倾角人工晶体术后后囊混浊(PCO)情况。方法选取2013年4月至2017年4月间在西安交通大学第一附属医院和西安市第一医院进行白内障超声乳化摘除手术并植入人工晶状体老年性白内障患者100例,依据自愿原则,29例患者植入后倾角为5°FLEX晶体,作为FLEX组;38例患者植入倾角为0°AO晶体,作为AO组;33例患者植入前倾角为10°SKY晶体,作为SKY组;观察患者术后眼底、并发症、发生后囊混浊及屈光度、前房深度变化情况。结果随访期间三组患者都没有发生Ⅲ级后囊混浊,三组患者在术后1个月、3个月及5个月发生PCO几率对比差异无统计学意义(P>0.05);三组患者术后5个月随访时,屈光度对比差异无统计学意义(P>0.05);三组对象手术前后前房深度对比差异均无统计学意义(P>0.05);患者手术中均未出现后弹力层脱落、切口撕裂、后囊膜破裂、角膜内皮损坏及玻璃体溢出等,患者均未发生黄斑囊样水肿、切口渗漏、视网膜脱落、人工晶体偏位青光眼或继发眼内感染等。结论人工晶状体为后倾角和常规人工晶状体对患者术后后囊混浊影响无明显差异。Objective To study the posterior capsule opacification（PCO） after phacoemulsification and intraocular lensimplantation. Methods From April 2013 to April 2017, in the First Affiliated Hospital of Xi’an Jiaotong University andXi’an No. 1 Hospital, 100 senile cataract patients were undergone phacoemulsification surgery and intraocular lensimplantation; according to the principle of voluntary, of which, 29 patients after implantation for angle of 5° FLEX crystalwere selected as the FLEX group; 38 patients were implanted with an inclination of 0° AO crystal as the AO group; 33 patients were implanted before angle of 10° SKY crystal as the SKY group. Postoperative complications, fundus, posteriorcapsule opacification and diopter, anterior chamber depth changes were observed. Results There were no grade Ⅲposterior capsule opacification in the three groups during the follow-up period. There was no significant difference in theincidence of PCO between the three groups at 1 month, 3 months and 5 months after operation（P〉0.05）. There was nosignificant difference in diopter contrast between the 5 months follow-up（P〉0.05）. There was no significant difference in thedepth of anterior chamber between the three groups before and after surgery（P〉0.05）. There was no post-elastic layer in theoperation. Shedding, tearing of the incision, rupture of the posterior capsule, damage of the corneal endothelium and vitreousleakage did not occur in patients with cystoid edema, incision leakage, retinal detachment, intraocular lens deviationglaucoma or secondary intraocular infection. Conclusion There was no significant difference in the effect of intraocularlens with posterior tilt angle and conventional intraocular lens on postoperative posterior capsule opacification.
