超微血流显像引导浅表包块穿刺活检的临床价值

摘　　要：目的探讨超微血流显像引导下浅表包块穿刺活检的临床价值。方法选取2015年10月至2017年3月本院收治的经临床药物治疗或手术后病理证实的44例浅表包块患者进行超微血流成像检查,根据包块的微血流显示情况,选取穿刺病灶及取材路径,比较分析穿刺活检结果与手术后病理或临床药物治疗的结果,统计穿刺活检的成功率及诊断准确率。结果 44例浅表包块穿刺活检结果显示,良性包块27例(其中包括结核6例,炎细胞浸润2例),恶性包块16例,取材不佳1例。所有患者均无明显并发症发生。临床药物治疗或术后病理结果显示,1例炎细胞浸润手术后病理诊断为骨外黏液样软骨肉瘤,1例取材不佳手术后病理诊断为淋巴瘤,余42例与穿刺活检结果相吻合,其中6例结核、1例炎性包块患者经药物治疗后证实,19例良性包块、16例恶性包块经手术后病理证实。穿刺成功率为97.73%(43/44),诊断准确率为95.45%(42/44)。结论超微血流显像技术引导浅表包块穿刺活检安全可靠、诊断准确率高,对疾病的诊断及治疗具有较高的临床价值。Objective To discuss the clinic value of biopsy for superficial masses guiding by superb micro-vascular imaging. Method We selected 44 cases of superficial masses patients which were pathologically proven after surgery or were under treatment of medicine to receive superb micro-vascular imaging examination from October 2015 to March 2017. According to the micro-vascular images of masses, we chose the location and the route to puncture, and compared the biopsy result with the pathological result or clinical result of medicine treatment. Then we analyzed the puncture success rate and the diagnosing accuracy. Result Biopsy of these 44 cases of superficial masses showed： 27 cases of benign tumors, including 6 cases of tuberculosis and 2 cases of inflammatory cells infiltration; 16 cases of malignant masses and 1 case with poor material quality. All these cases had no complications. Pathological result or clinical result of medicine treatment： 1 case of inflammatory cells infiltration was confirmed to be extraskeletal myxoid chondrosarcoma by pathology after surgery, 1 case with poor material quality was histologically confirmed to be lymphoma, other 42 cases（6 cases of tuberculosis and 1 case of inflammation were diagnosed by medicine treatment, 19 cases of benign tumors and 16 cases of malignant masses were diagnosed by pathology） were consistent with the biopsy result. Puncture success rate was about 97.73%（43/44）, and diagnosing accuracy was about 95.45%（42/44）. Conclusion Needle biopsy for superficial masses guiding by superb micro-vascular imaging is safe, reliable and has a high diagnosing accuracy, so it is quite valuable for diagnosis and therapy.
