摘  要:目的:系统评价天麻钩藤饮与钙通道阻滞剂相比预防性治疗偏头痛的疗效及安全性。方法:电子检索Pub Med、Cochrane library、CNKI、VIP、万方和CBM,分别用Stata 14.0和TSA v0.9进行Meta分析和试验序贯分析。结果:共纳入10个RCT包括900例患者。结果显示与钙通道阻滞剂相比,天麻钩藤饮预防性治疗偏头痛的疗效增加31%(RR=1.31,95%CI(1.22,1.40))。试验序贯分析提示天麻钩藤饮治疗偏头痛的疗效显著,证据充分。治疗组不良反应发生率为3.1%,对照组为7.5%。结论:天麻钩藤饮预防性治疗偏头痛的疗效优于钙通道阻滞剂,具有较好的长期疗效,不良反应较少,仍需高质量、大样本的临床随机对照研究进一步支持验证。Objective: To systematically review the clinical effect and safety of Tianma Gouteng Yin(TGY) for migraine compared to Calcium channel blockers(CCB). Method: Such databases as Pub Med,Cochrane library,CNKI,VIP,Wanfang data and CBM were searched and we carried out meta-analysis and TSA analysis by using Stata 14 version and TSA v0. 9 respectively. Results: Totally 10 RCT including 900 cases were included eventually. The analysis shows that the total effective rate of prophylactic treatment of migraine in TGY increased by 31% compared to CCB(RR = 1. 31,95%CI(1. 22,1. 40)). The TSA analysis shows that the evidence of the total effective rate is definite. The incidence of adverse reactions was 3. 1% in the treatment group and 7. 5% in the control group. Conclusion: The effectiveness and safety of TGY in the treatment of migraine is better than CCB,which has a good long-term effect. But a larger sample,higher quality RCT test are definitely needed for confirming the conclusion.
