急性胰腺炎腹痛部位的临床意义分析

摘　　要：目的:探讨急性胰腺炎入院时腹痛部位的临床意义。方法:选择广西医科大学第一附属医院消化内科2013年全年急性胰腺炎住院患者,收集腹痛部位、病因、病情严重程度等临床数据,进行回顾性分析。结果:纳入94例急性胰腺炎患者,其中轻症41例,中度重症44例,重症9例,胆源性胰腺炎38例。65例患者疼痛部位在上腹部、中上腹、左上腹,单独或伴有右上腹痛者9例,病因均为胆源性,脐区2例、右下腹1例及无痛患者2例,病因均为特发性,全腹痛者15例,各组病因学分布比较有统计学差异(P=0.02)。全腹痛组与非全腹痛组比较多存在腹膜炎(P<0.001),病情更严重(P=0.002)。结论:入院时的疼痛部位对急性胰腺炎的病因有提示作用,有右上腹痛者提示为胆源性,其余非典型疼痛部位或无痛提示为特发性,而全腹痛者病情更严重。Objective：To explore the clinical significance of abdominal pain in a cute pancreatitis.Methods：The patients with acute pancreatitis in the department of digestive medicine of ahospital in 2013 were selected.The clinical data of abdominal pain site,etiology,severity of illness and other clinical data were collected and analyzed retrospectively.Results：94 patients with acute pancreatitis were admitted.Among them,41 were mild,44 were moderate severe,9 were severe,and 38 were biliary pancreatitis.The painsites of 65 patients were in the upper abdomen,middle upper abdomen and left upper abdomen,and 9 patients were separately or accompanied by right upper abdominal pain.The etiology was biliary.2 cases of umbilical area,1 case of right lower abdomen and 2 cases of pain less patients,and thee tiology is idiopathic;Total abdominal pain occurred in 15 cases.The etiological distribution of each group was statistically different（P=0.02）.Conclusion：The pain site at the time of admission has a suggestive effect on the etiology of acute pancreatitis.Patients with right upper abdominal pain are suggested to bebiliary,while those with other atypical pain or no pain are suggested to be idiopathic,while those with total abdominal pain are more serious.
