女性阴道中肾管肿瘤临床病例观察

摘　　要：目的:探讨女性阴道中肾管肿瘤的临床病理学特征、诊断和鉴别诊断,以提高对该肿瘤的认识。方法:对一例阴道中肾管肿瘤进行常规病理检查、HE、组织化学及免疫组织化学染色,显微镜下观察,并结合文献进行讨论。结果:患者,女,32岁,体检发现阴道肿块一月。巨检:阴道壁多发乳头样突起,质地细嫩,切面淡黄色。显微镜下:肿瘤细胞呈不同方式排列:立方状、柱状,核分型易见,腺体增生呈管状结构,腺腔内可见透明的嗜酸性分泌物,小管内皮由立方的和扁平细胞组成。免疫组织化学:CEA阳性,Ki-67部分阳性,CD10、EMA、ER、PR均为阴性。结论:女性阴道中肾管肿瘤是一种非常罕见的肿瘤,临床症状缺乏特异性,极易误诊。诊断主要依靠组织病理学及免疫组织化学,并应与支持-间质细胞肿瘤、子宫内膜样腺癌、粒层细胞瘤和腺瘤样瘤等其他妇科肿瘤进行鉴别诊断。Objective： To investigate the clinical pathology of tumor in the female vagina nephridia feature,diagnosis and differential diagnosis,to improve the understanding of the tumor.Methods： Routine pathological examination,HE,histochemical and immunohistochemical staining of tubular case of vaginal tumor by microscopic observation,and combine the literature to carry on the discussion.Results： The female patient,aged 32,was found to have a vaginal mass for one month.Gross specimen： Vaginal wall many nipple like process,delicate texture,section pale yellow.Microscopically： The tumor cells are arranged in different ways： Cuboidal and columnar,nuclear type is easy to see,gland hyperplasia is tubular structure,a clear,eosinophilic exudate were seen in the lumen,and the tubular endothelium consists of cuboidal and squamous cells.Immunohistochemistry： CEA positive,Ki-67 partial positive,CD10,EMA,ER and PR were negative.Conclusion： The female vagina tumor nephridium is a rare tumor,the clinical symptoms are lack of specificity and are easily misdiagnosed.The diagnosis mainly depends on histopathology and immunohistochemistry,differential diagnosis should be made between other stromal tumors such as stromal cell tumor,endometrioid adenocarcinoma,granulosa cell tumor and adenomatoid tumor.
