摘 要:目的观察分析23G硅油取出术后发生高眼压的主要原因及相关危险因素。方法回顾性分析2015年2月至2016年1月在我院就诊行23G硅油取出术患者的临床资料及术后眼压的变化,探讨术后高眼压的可能原因。根据患者是否出现高眼压,分为高眼压组和无高眼压组,分析两组患者的性别、术后低眼压、眼内再出血、联合白内障手术、术前高眼压和合并有高度近视的特征进行统计分析。结果 114例(114只眼)23G硅油取出术后患者中,39例患者出现术后高眼压,术后高眼压发生率34.2%。引起高眼压发生的危险因素包括硅油取出术后早期低眼压(χ~2=22.47,P<0.001)、硅油取出术后眼内再出血发生(χ~2=6.77,P=0.009)。结论 23G硅油取出术后高眼压可能的原因有术后早期的低眼压、术后眼内出血、晶状体摘除、术前高眼压,其中术后低眼压和眼内再出血与术后高眼压的发生关系密切,因此术后需要密切监测眼压,提高手术质量根据病因预防控制。Objective To investigate the main causes and related risk factors of high intraocular pressure after 23 G silicone oil removal. Methods The clinical data and the reasons for changes of intraocular pressure of patients( IOP) after23 G silicone oil removal in our hospital from February 2015 to January 2016 were analyzed retrospectively. High intraocular pressure and no high intraocular pressure group were divided according to the postoperative high intraocular pressure. Factors analyzed included gender,postoperative low intraocular pressure,intraocular rebleeding,combined cataract surgery,preoperative high intraocular pressure and high myopia. Results 114 eyes were collected. High intraocular pressure after23 G silicone oil removal occurred in 39 patients,and the incidence of postoperative high intraocular pressure was 34. 2%.The risk factors for high intraocular pressure included early low intraocular pressure after the removal of silicone oil( χ^2=22. 47,P 0. 001),and intraocular rebleeding after silicone oil removal( χ^2= 6. 77,P = 0. 009). Conclusions The possible causes of high intraocular pressure after 23 G silicone oil removal are early low intraocular pressure after surgery,intraocular rebleeding,cataract? extraction and high intraocular pressure before operation. Postoperative early low intraocular pressure and intraocular rebleeding are closely related to the occurrence of postoperative high intraocular pressure.Therefore,close monitoring of intraocular pressure is necessary to improve the quality of surgery.
