迷走神经电刺激联合中医综合理疗对脑卒中后病人偏瘫肢体功能恢复的效果研究

摘　　要：目的探讨迷走神经电刺激联合中医综合理疗对脑卒中后病人偏瘫肢体功能恢复的效果。方法选取2014年1月—2016年1月我院就诊诊治的120例脑卒中后肢体偏瘫病人作为研究对象,按照随机数表法分为A组、B组、C组各40例。A组给予迷走神经电刺激治疗,B组给予中医综合理疗,C组给予迷走神经电刺激联合中医综合理疗。治疗3个月后,比较3组病人偏瘫肢体功能恢复情况和治疗有效率。结果治疗后A组、B组、C组病人Fugl-Meyer上下肢功能指数量表评分(FMA评分)、Barthel指数量表评分(MBI评分)均高于治疗前,神经功能缺损评分低于治疗前,差异均有统计学意义(P<0.05)。治疗后,A组、B组FMA评分及MBI评分均明显低于C组,神经功能缺损评分高于C组,差异均有统计学意义(P<0.05);C组临床总有效率高于A组、B组,差异均有统计学意义(P<0.05)。结论在脑卒中后肢体偏瘫病人的肢体功能康复治疗中,联合运用迷走神经电刺激与中医综合理疗能更有效地改善偏瘫病人的肢体功能,增强病人日常生活自理能力,提高临床治疗有效率。Objective To investigate the effect of vagus nerve stimulation and Chinese medicine comprehensive physiotherapy on the recovery of limb function in patients with hemiplegia after stroke. Methods One hundred and twenty patients with hemiplegia after stroke in our hospital from January 2014 to January 2016 were selected and randomly divided into 3 groups：group A （ n =40） treated with vagus nerve stimulation,group B （ n =40） treated with Chinese medicine comprehensive physiotherapy,and group C （ n =40） treated with vagus nerve stimulation and Chinese medicine comprehensive physiotherapy for 3 months.After the treatment of 3 months,the recovery of limb function and total effective rate,Modified Edinburgh Scandinavian Stroke Scale （MESSS） were observed and compared. Results After treatment,the scores of Modified Barthel Index （MBI） and Fugl Meyer Assessment （FMA） increased while the scores decreased in 3 groups （ P 〈0.05）.The scores of FMA and MBI in group A and group B were significantly lower than that in group C,while MESSS scores in group A and group B were significantly higher than that in group C（ P 〈0.05）.After treatment,the total clinical effective rate in group C was higher than that in group A and group B （ P 〈0.05）. Conclusion Vagus nerve stimulation and Chinese medicine comprehensive physiotherapy can effectively improve the limb function,enhance the self care ability of daily life,improve the efficiency of clinical treatment in patients with hemiplegia after stroke.
