富马酸卢帕他定治疗过敏性鼻炎的临床疗效观察

摘　　要：目的探讨富马酸卢帕他定片口服治疗对过敏性鼻炎患者血清免疫球蛋白及Th1/Th2平衡的影响。方法选取该院2015年12月至2017年3月门诊就诊的164例过敏性鼻炎患者,依据随机数字表法分为观察组和对照组,各82例。对照组口服盐酸西替利嗪片,观察组口服富马酸卢帕他定片。观察两组的临床疗效、不良反应,并于治疗前后检测血清总IgE(tIgE)及特异性IgE(sIgE)水平及Th1和Th2细胞相关因子[包括肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-4、IL-5、IL-12]变化。结果观察组治疗总有效率为95.0%,高于对照组(χ2=10.982,P<0.05)。两组患者治疗后血清tIgE、sIgE水平下降,且观察组下降更明显(P<0.05)。两组患者治疗后血清TNF-α、IL-12水平较治疗前升高(P<0.05),IL-4、IL-5水平较治疗前降低(P<0.05),且观察组上述指标改变较对照组更明显(P<0.05)。观察组不良反应率为2.4%,低于对照组(χ2=5.754,P<0.05)。结论富马酸卢帕他定能够改善过敏性鼻炎患者的Th1/Th2失衡,促进Th1/Th2平衡向Th1倾斜,抑制炎性反应,提高过敏性鼻炎治疗效果。Objective To investigate the effect of rupatadine fumarate tablet oral therapy on allergic rhinitis patients for serum immunoglobulin and the balance of Th1/Th2. Methods A total of 164 cases of allergic rhinitis patients in hospital from December 2015 to March 2017 were divided into the observation group and the control group according to the random number table,with 82 cases in each group.The control group was treated with cetirizine hydrochloride tablets,the observation group received rupatadine fumarate tablets.The clinical curative effect and side effect were observed.The levels of total IgE （tIgE）,specific IgE （sIgE） and changes of Th1 and Th2 cell related factors [including tumor necrosis factor-α （TNF-α）,interleukin （IL）-4,IL-5,IL-12] were recorded before and after the treatment. Results The total effective rate of the observation group was 95.0%,which was significantly higher than that of the control group （ χ 2=10.982, P 〈0.05）.The levels of tIgE and sIgE decreased after the treatment in both groups,which decreased more significantly in the observation group （ P 〈0.05）.After the treatment,the levels of TNF-α、IL-12 increased significantly （ P 〈 0.05）,while IL-4,IL-5 levels decreased （ P 〈0.05）,and the changes of those indices in the observation group were more remarkable （ P 〈0.05）.The side effect rate of the observation group was 2.4%,which was significantly lower than that of the control group （ χ 2=5.754, P 〈0.05）. Conclusion Rupatadine fumarate can keep balance of Th1/Th2 and improve the therapeutic effect by inhibiting the inflammatory reaction.
