摘 要:目的探讨分析血清C反应蛋白(CRP)、D-二聚体(D-D)及降钙素原(PCT)水平变化与急性胰腺炎病情变化的相关性及其临床价值。方法回顾性分析180例急性胰腺炎患者的临床资料,根据患者病情将其分为重症组、中度组和轻度组,各60例。比较三组患者的血清CRP、D-D、PCT水平、严重程度床边指数(BISAP)评分及CT严重程度指数(CTSI)评分,采用非参数Spearman等级相关分析进行相关性分析。结果中度组和重症组患者的血清CRP、D-D及PCT水平、BISAP及CTSI评分显著高于轻度组,且重症组患者的血清CRP、D-D及PCT水平、BISAP及CTSI评分显著高于中度组,差异具有统计学意义(P<0.05)。急性胰腺炎患者BISAP及CTSI评分与血清CRP、D-D及PCT水平均正相关(P<0.05)。结论血清CRP、D-D及PCT水平变化与急性胰腺炎病情变化有着密切的相关性,其可用于评估急性胰腺患者的病情分级及治疗效果。Objective To discuss and analyze the clinical value and correlation between changes of serum C-reactive protein(CRP), D-dimer(D-D) and procalcitonin(PCT) levels and acute pancreatitis. Methods The clinical data of 180 patients with acute pancreatitis were retrospectively analyzed. According to the patients' condition, they were divided into severe group, moderate group and mild group, with 60 cases in each group. The levels of serum CRP, D-D and PCT,scores of severity bedside index(BISAP), CT severity index(CTSI) were compared among the three groups, and the nonparametric Spearman rank correlation analysis was used for correlation analysis. Results The levels of serum CRP, DD and PCT, scores of BISAP and CTSI in the moderate group and severe group were higher than those in the mild group,and those of the severe group were higher than the moderate group(P〈0.05). The BISAP and CTSI scores of patients with acute pancreatitis were positively correlated with serum CRP, D-D, and PCT levels. Conclusion The changes of serum CRP, D-D and PCT levels are closely related to the changes of acute pancreatitis, which can be used to evaluate the severity and treatment effect of patients with acute pancreatitis.
