复合乳酸菌胶囊辅助治疗肝硬化自发性细菌性腹膜炎的临床疗效研究

摘　　要：目的:研究复合乳酸菌胶囊对肝硬化自发性细菌性腹膜炎的辅助治疗作用。方法:96例肝硬化自发性细菌性腹膜炎患者随机分为复合乳酸菌治疗组和常规治疗组,每组48例;另外选取48例肝硬化腹水非自发性细菌性腹膜炎患者作为对照组。常规治疗组采用抗感染、补充白蛋白、保肝、利尿等常规治疗。复合乳酸菌治疗组在常规综合治疗的基础上加用复合乳酸菌胶囊口服。观察3组患者治疗前后血浆降钙素原(PCT)、白介素-6(IL-6)、C反应蛋白(CRP)的变化。并观察复合乳酸菌治疗组和常规治疗组患者治疗7 d后的疗效。结果:自发性细菌性腹膜炎组血浆PCT、IL-6和CRP水平明显高于对照组,差异具有统计学意义(P<0.05)。复合乳酸菌治疗组治疗后血浆PCT、IL-6和CRP水平较常规治疗组明显下降,差异有统计学意义(P<0.05)。复合乳酸菌治疗组和常规治疗组的临床总有效率差异有统计学意义(93.75%vs 72.92%,P<0.05)。结论:复合乳酸菌胶囊辅助治疗肝硬化自发性细菌性腹膜炎,可调节肠道菌群,改善肠道功能,减轻炎症反应,降低血浆中PCT、IL-6和CRP的水平。Objective： To investigate the clinical effect of lactobacillus complex capsules as the adjuvant therapy for cirrhotic patients with spontaneous bacterial peritonitis. Methods： A total of 96 cirrhotic patients with sponta-neous bacterial peritonitis were randomly divided into lactobacillus complex capsule treatment group and convention-al treatment group, with 48 patients in each group, and another 48 cirrhotic ascites patients without spontaneous bac-terial peritonitis were enrolled as control group. The patients in the conventional treatment group were given conven-tional treatment including anti-infective therapy, albumin supplementation, liver protection, and diuretic therapy,and those in the lactobacillus complex capsule treatment group were given oral lactobacillus complex capsules in ad-dition to the conventional treatment. The serum levels of procalcitonin（PCT）, interleukin-6（IL-6）, and C-reactiveprotein（CRP） were measured before and after treatment and compared between these groups. The response rate after7 days of treatment was compared between the lactobacillus complex capsule treatment group and the conventionaltreatment group. Results： The cirrhotic patients with spontaneous bacterial peritonitis had significantly higher se-rum levels of PCT, IL-6, and CRP than those without spontaneous bacterial peritonitis（P〈0.05）. After treatment,the lactobacillus complex capsule treatment group had significantly lower serum levels of PCT, IL-6, and CRP thanthe conventional treatment group（P〈0.05）. There was a significant difference in overall response rate between thelactobacillus complex capsule treatment group and the conventional treatment group（93.75% vs 72.92%, P〈0.05）.Conclusion： As the adjuvant therapy for cirrhotic patients with spontaneous bacterial peritonitis, lactobacillus com-plex capsules can regulate gut microbiota, improve intestinal function, alleviate inflammatory response, and reduce serum levels of PCT, IL-6, and CRP.
