摘  要:目的多层螺旋CT各向同性扫描行多平面重组,重建耳蜗标准方位图像并测量正常耳蜗HRCT相关径线,建立正常值范围,探讨其在耳蜗发育异常诊断中的价值。方法收集50例(100耳)正常人颞骨HRCT扫描图像,用MPR方法对图像标准化处理,重建标准方位图像Ⅰ和Ⅱ,在标准方位图像上设计并测量耳蜗9个结构径线,记录数据并分析各年龄组、性别、左右侧之间各径线大小有无差异,并建立正常值范围。结果 1)正常人耳蜗各径线之间无年龄、性别、左右侧之间的差异;2)正常人耳蜗各径线正常值为:A (6. 13±0. 24)mm,B (4. 49±0. 21)mm,C (2. 77±0. 17)mm,D (3. 69±0. 23) mm,E (3. 9±0. 5) mm2,F (2. 32±0. 25)mm,G (0. 99±0. 21)mm,H (9. 07±0. 25)mm,I (2. 28±0. 13)mm。结论在经过标准化处理的MPR图像上测量耳蜗径线更具科学性;径线测量可辅助诊断耳蜗发育异常。Objective To reconstruct the standard position images of cochlea using multi-planar reformation(MPR) of high resolution computed tomography(HRCT) and to establish normative measurements of cochlea on standard position images so as to aid in the diagnosis of cochlea hypolasia. Methods HRCT scanning of temporal bone was performed in 50 volunteers(100 ears)with normal hearing and without any disease in the middle and inner ear. The images of all cases were postprocessed using MPR.Two standard position images(positionⅠ andⅡ) were reconstructed. 9 structures of cochlea(named A-I) were measured on the images of position Ⅰ and Ⅱ. Statistical analysis was done on different age groups,male and female groups,left and right groups in SPSS program. Normative data of 9 measurements were established. Results 1) All measurements of cochlea have no difference in different age groups,male and female groups,left and right groups; 2) The normative data of cochlea measurements were as follows: A(6. 13 ± 0. 24) mm,B(4. 49 ± 0. 21) mm,C(2. 77 ± 0. 17) mm,D(3. 69 ± 0. 23) mm,E(3. 9 ± 0. 52)mm2,F(2. 32 ± 0. 25) mm G(0. 99 ± 0. 21) mm,H(9. 07 ± 0. 25) mm,I(2. 28 ± 0. 13) mm. Conclusion The measurements of cochlea is more accuracy on standard position images reconstructed using multi-planar reformation and is helpful to identify the abnormalities of cochlea.
