基质金属蛋白酶-9(MMP-9)及其组织抑制因子-1(TIMP-1)在结直肠癌治疗前后的表达及其意义

摘　　要：目的探讨基质金属蛋白酶-9(MMP-9)及其组织抑制因子-1(TIMP-1)在结直肠癌治疗前后的表达及其意义。方法选取行手术治疗的结直肠癌患者46例为研究对象。患者术前、术后7 d、化疗后7 d时,均于清晨空腹抽取静脉血4 m L,Elisa法检测血浆中MMP-9、TIMP-1水平,并分析其与TNM分期、组织分化程度、性别、年龄、肿瘤发生部位、肿瘤直径等的关系。比较术前、术后7 d、化疗后7 d期间血浆MMP-9、TIMP-1水平及MMP-9/TIMP-1比值。结果不同TNM分期患者,MMP-9、TIMP-1水平、MMP-9/TIMP-1比值存在明显差异,随着分期的增加,血浆MMP-9、TIMP-1水平及MMP-9/TIMP-1比值呈逐渐升高趋势,差异均有统计学意义(P<0.05);低分化组血浆MMP-9、TIMP-1水平及MMP-9/TIMP-1比值明显高于高、中分化组,差异有统计学意义(P<0.01)。术后7 d、化疗后7 d时,血浆MMP-9、TIMP-1水平及MMP-9/TIMP-1比值均明显低于术前,差异有统计学意义(P<0.01);化疗后7 d时,血浆MMP-9、TIMP-1水平及MMP-9/TIMP-1比值均明显低于术后7 d,差异有统计学意义(P<0.01)。结论 MMP-9、TIMP-1与结直肠癌TNM分期及肿瘤侵袭性密切相关,血清中MMP-9、TIMP-1水平检测可能成为结直肠癌病情评估的血清学指标。Objective To explore the expression and significance of MMP-9 and TIMP-1 in colorectal cancer before and after treatment. Methods 46 cases of colorectal cancer patients were selected. Before and after surgery 7 d,7 d after chemotherapy,4 m L of venous blood was extracted in the early morning. Elisa method was used to detect MMP-9 and TIMP-1 level in plasma. The relationship with TNM stage,tissue differentiation,gender,age,tumor location and tumor diameter was analyzed. Compare plasma MMP-9 and TIMP-1 level and MMP-9/TIMP-1 ratio before and after surgery 7 d,7 d after chemotherapy. Results Patients with different TNM staging,MMP-9 and TIMP-1 level,MMP-9/TIMP-1 ratio had significant differences,with the increase of installment,the plasma levels of MMP-9 and TIMP-1 and MMP-9/TIMP-1 ratio showed a trend of increasing gradually,the comparative differences has statistically significance（ P〈0. 05）. Poorly differentiated group of plasma levels of MMP-9 and TIMP-1 and MMP-9/TIMP-1 ratio were significantly higher than that of high,middle differentiation group,by the comparative difference was statistically significant（ P〈 0. 01）. After surgery and chemotherapy 7 d,the plasma levels of MMP-9 and TIMP-1 and MMP-9/TIMP-1 ratio were significantly lower than preoperative,comparative difference has statistically significance（ P〈 0. 01）. 7 d after chemotherapy,the plasma levels of MMP-9 and TIMP-1 and MMP-9/TIMP-1 ratio were significantly lower than after surgery 7 d,the comparative difference was statistically significance（ P 0. 01）. Conclusion MMP-9 and TIMP-1 with colorectal cancer is closely related to the TNM staging and tumor invasive,MMP-9 and TIMP-1 level in blood test could become colorectal cancer illness serological indexes of evaluation.
