卵巢成熟性囊性畸胎瘤的超声诊断与病理对照分析

摘　　要：目的探讨超声对卵巢成熟性囊性畸胎瘤(MCT)的超声诊断价值。方法回顾性分析2014年6月至2017年11月在奉贤区中心医院妇科住院的152例158个卵巢MCT的超声表现,将超声与病理结果进行对照分析。结果卵巢MCT超声表现与肿瘤大体结构密切相关。由脂质毛发构成者呈面团征;由脂质和多量液体构成者呈脂液分层征;由脂质、毛发、骨组织等构成者表面呈高-强回声,后方回声渐次衰减,似瀑布状;囊壁上可见强回声结节,后方伴声影呈壁立结节状;囊内见多量毛发时呈线条征;大部分为脂质,当其受压弥散时呈星花征;囊内成分多时,表现为杂乱结构征。158个卵巢MCT超声诊断与术后病理诊断完全符合125个、误诊10个、不完全符合14个、漏诊9个。结论超声检查对卵巢MCT具有较高的诊断价值,其回声类型与瘤内成分类型及其含量密切相关,而与成分类型多寡并无对应关系。通过声像图表现与病理对照分析可提高诊断准确率,为临床治疗提供诊断依据。Objective To explore the ultrasonic diagnostic value of ovarian mature cystic teratoma（MCT）.Methods The ultrasonographic findings of 158 ovaries mature cystic teratoma in 152 cases of gynecology hospitalized in Department of Ultrasound Medicine of the Central Hospital of Shanghai Fengxian District from June 2014 to November2017 were retrospectively analyzed. The results of ultrasound and pathology were compared and analyzed. Results The ultrasonic manifestations of teratoma were closely related to the gross structure of the tumor. The tumor composed of lipid hairs showed dough sign; those with lipid and multiple liquid components were lipid stratified; those composed of lipid,hair, bone tissue was high-strong echoed on the surface, with the echo attenuated gradually in the rear, like a waterfall, and a strong echo nodule was seen on the wall of the capsule, and the posterior acoustical shadow appeared as a wall standing nodule; the tumor with large number of hairs showed line sign; the tumor mostly composed of lipid showed signs of star when they were pressed diffusedly; the tumor with multiple components showed signs of disorder. Among the 158 teratoma, the ultrasonographic diagnosis of 125 cases were fully conformed to postoperative pathological diagnosis, with 10 cases of misdiagnosis, 14 cases of incomplete coincidence and 9 cases of missed diagnosis. Conclusion Ultrasound examination has a high diagnostic value for mature cystic teratoma. The type of echo is closely related to the type and content of intratumoral components, but has no corresponding relationship with the composition type. The accuracy of diagnosis can be improved by the contrast analysis of image and pathology, which provides diagnostic basis for clinical treatment.
