翼状胬肉切除术中应用纤维蛋白胶对术后眼表修复分析

摘　　要：目的研究翼状胬肉切除联合自体结膜瓣移植术中应用医用纤维蛋白胶对术后角膜修复及疼痛评分的影响。方法收集60例单眼原发性鼻侧翼状胬肉,随机分为A,B组,各30例,由同一专家行翼状胬肉切除联合自体结膜瓣移植术,其中A组采用医用纤维蛋白胶粘合固定移植片,B组采用10-0尼龙缝线固定移植片,术后进行随访,观察手术时间、术后疼痛不适及角膜上皮修复情况。结果 A组手术时间(24.3±5.3min)较B组(33.1±6.2min)短,两组之间差异具有统计学意义(P<0.05)。术后A组患者疼痛不适及异物感较对照组轻,两组间差异有统计学意义(P<0.05)。术后3d及7d角膜荧光染色评分试验组优于对照组,具有统计学意义(P<0.05)。结论在翼状胬肉切除联合自体结膜瓣移植术中应用医用纤维蛋白胶粘合结膜移植瓣安全有效,可以减少手术时间,缓解术后眼部不适。Objective To study the effect of medical fibrin glue on postoperative corneal repair and pain scores in pterygium excision combined with autologous conjunctival flap transplantation. Methods Sixty cases of primary unilateral nasal pterygium were randomly divided into group A（ n = 30） and group B（ n = 30）. Pterygium excision combined with autoconjunctival flap transplantation was performed by the same expert. Group A was treated with medical fibrin glue,and group B was fixed with 10-0 nylon suture. Follow-ups were performed to observe the operation time,postoperative pain discomfort,and repair of corneal epithelium. Results The operation time of the A group was（ 24. 3 ± 5.3 min） shorter than that of the B group（ 33. 1 ± 6. 2 min）. The difference between the two groups was statistically significant（ P〈0. 05）. The pain and foreign body sensation in the A group were lighter than those in the B group. The difference between the two groups was statistically significant（ P〈0. 05）. The scores of corneal fluorescence staining at 3 days and 7 days after surgery were better in the test group than in the control group（ P〈0. 05）. Conclusion The use of fibrin glue-conjunctival flap in the treatment of pterygium excision combined with autologous conjunctival flap transplantation is a safe and effective way to reduce the operation time and relieve postoperative eye discomfort.
