摘　　要：目的探讨ABCD^2评分联合颈部血管超声对短暂性脑缺血发作(TIA)后7d内发生脑梗死的预测价值。方法选择颈动脉系统TIA患者80例,并根据ABCD^2评分及颈部血管超声结果进行分组,比较各组患者TIA后7d内脑梗死发生率。结果 80例TIA患者7d内进展为脑梗死18例(22.5%),低、中、高危组7d内进展为脑梗死的比例分别为6.9%,27.3%,38.9%,组间比较差异有统计学意义(P<0.05)。颈动脉超声斑块组和无斑块组进展为脑梗死发生率分别为47.1%,4.3%(P<0.05)。当ABCD^2评分≥4分时,颈动脉超声斑块组脑梗死发生率为54.2%,高于颈动脉超声无斑块组(11.1%)(P<0.05)。结论 ABCD^2评分结合颈动脉超声能提高短期内TIA患者脑梗死的预见风险。Objective To investigate the value of ABCD^2 score combined with carotid ultrasound in predicting cerebral infarction after transient ischemic attack（ TIA） at 7 d. Methods Eighty patients with TIA were selected and divided into groups according to ABCD^2 score and carotid ultrasound,the incidence of cerebral infarction within 7 days after TIA in each group was compared. Results In 80 patients with TIA,18 patients developed cerebral infarction in 7 days,The proportion of cerebral infarction in the low,middle and high risk group within 7 days were 6. 9% and 27. 3%,38. 9% respectively. The difference between each group was statistically significant（ P〈0. 05）. The incidence of cerebral infarction in carotid plaque group and non-plaque group were 47. 1%,4. 3%（ P〈0. 05）. In ABCD^2 score ≥4 group,the incidence of cerebral infarction in carotid plaque group was 54. 2%,higher than that in non-plaque group（ 11.1%）（ P〈0. 05）. Conclusion The ABCD^2 score combined with carotid ultrasound can develop the predicted risk of cerebral infarction after TIA in the short term.
