摘  要:目的探讨呼吸训练对蛛网膜下腔出血患者肺功能的影响。方法选取2017年1月~2017年10月入院的蛛网膜下腔出血患者共50例。随机分为两组,呼吸训练组和常规治疗组。经训练后观察患者呼吸频率、肺部感染情况、住院天数、肺功能(最大通气量百分比、用力肺活量、血氧饱和度、动脉血氧分压)、呼吸力学指标(气道峰压、呼吸压力)等情况。结果呼吸训练组患者呼吸频率、肺部感染、住院天数均好于常规训练组患者,两者差异有统计学意义(P<0.05);呼吸训练组最大通气量百分比(61.52%±1.42%)、用力肺活量(3.11±0.05L)、血氧饱和度(93.85%±1.15%)、动脉血氧分压(89.61±0.30mmHg)、气道峰压(32.26±0.70cm H_2O)、呼吸压力(28.67±0.78cm H_2O)与常规治疗组(依次为53.76%±1.56%,3.01±0.145L,92.64%±0.84%,87.89±0.58mmHg,33.29±0.24cm H_2O,32.08±1.02cm H_2O)比较差异均非常显著(P<0.01)。结论呼吸训练可明显改善蛛网膜下腔出血患者肺功能恢复,减少并发症的发生,有效改善患者预后。Objective To explore the effect of respiratory training on pulmonary function in patients with subarachnoid hemorrhage. Methods Fifty patients with subarachnoid hemorrhage admitted from January 2017 to October 2017 were selected. They were randomly divided into two groups,breathing training group and conventional treatment group. After training,the respiratory rate,pulmonary infection,hospitalization days,lung function test: the maximum percentage of ventilation,forced vital capacity,patient oxygen saturation,arterial oxygen pressure,respiratory mechanics indicators: airway peak Pressure,respiratory pressure situation were observed. Results The respiratory rate,lung infection,and length of hospital stay in the breathing training group were all better than those in the conventional training group.The difference was statistically significant( P〈0. 05). The percentage of maximum ventilation( 61. 52% ± 1. 42%),forced vital capacity( 3. 11 ± 0. 05 L),oxygen saturation( 93. 85% ± 1. 15%),partial pressure of arterial oxygen( 89. 61± 0. 30 mmHg),peak airway pressure( 32. 26 ± 0. 70 cm H_2 O),respiratory pressure( 28. 67 ± 0. 78 cm H_2 O) in the breathing training group and in conventional treatment groups( 53. 76% ± 1. 56%,3. 01 ± 0. 145 L,92. 64% ± 0. 84%,87. 89± 0. 58 mmHg,33. 29 ± 0. 24 cm H_2 O,32. 08 ± 1. 02 cm H_2 O,respectively). The difference was statistically significant( P〈0. 01). Conclusion Breathing training can significantly improve pulmonary function recovery in patients with subarachnoid hemorrhage,reduce the incidence of complications,effectively improve the prognosis of patients.
