摘  要:目的 探讨情绪管理对不同心功能分级心力衰竭(简称心衰)患者营养风险的影响.方法 选取2015年9月—2016年9月于河南科技大学第一附属医院心内科住院治疗的心功能Ⅱ-Ⅳ级的心衰患者180例,根据入院的先后顺序分为对照组和观察组各90例,对照组施行常规心理护理,观察组在常规心理护理基础上施行情绪管理.于入院第1天和出院前1 d分别采用中文版简式-简明心境问卷(POMS-SF)和NRS 2002评估患者的情绪状况和营养评分状况.结果 入院第1天,两组不同心功能分级心衰患者POMS-SF评分比较,差异无统计学意义(t值分别为-0.560、0.929、-0.065;P〉0.05).出院前1 d,观察组不同心功能分级心衰患者POMS-SF评分低于对照组,差异有统计学意义(t值分别为-2.808、-2.322、-2.445;P〈0.05).入院第1天,两组不同心功能分级心衰患者NRS 2002评分比较,差异无统计学意义(t值分别为0.173、-0.362、-0.392;P〉0.05).出院前1 d,观察组不同心功能分级心衰患者NRS 2002评分低于对照组,差异有统计学意义(t值分别为-3.791、-3.372、-6.542;P〈0.05).结论 情绪管理可以降低不同心功能分级的心衰患者的情绪状况和营养风险,效果好于常规的心理护理,值得临床推广应用.Objective To explore the influence of emotion management on nutrition risk in heart failure patients with different heart function. Methods A total of 180 heart failure patients with New York Heart Association (NYHA) heart function class Ⅱ, Ⅲ and Ⅳ who were hospitalized in Cardiovascular Department of the First Affiliated Hospital of He'nan University of Science and Technology from September 2015 to September 2016 were collected in this study. The participants were divided into two groups according to the order of admission. The control group (n=90) received routine psychological nursing, while the observation group (n=90) received additional emotional management. The Profile of Mood States-Short Form(POMS-SF) and Nutrition Risk Screening Tool 2002(NRS 2002) were used to assess the emotional state and nutritional status of the patients on the first day of admission and one day before discharge. Results On the first day of admission, there were no significant differences in the scores of POMS-SF(t=-0.560, 0.929, -0.065; P 〉 0.05)and NRS 2002(t=0.173, -0.362, -0.392 ; P 〉 0.05) of heart failure patients with different heart function between two groups. But on one day before discharge, the intervention group had significantly lower POMS-SF score(t=-2.808, -2.322, -2.445; P 〈 0.05) and lower NRS 2002 score (t=-3.791, -3.372, -6.542; P 〈 0.05) compared with the control group. Conclusions The emotional management can effectively improve the emotional state and reduce the nutritional risk in heart failure patients with different functional classification. It has a better effect compared with conventional psychological nursing and is worthy of clinical application.
