摘  要:目的评价心肺复苏后昏迷患者应用新型肠内营养液恒温装置的效果及对营养支持并发症的影响。方法选择2017年6月至2018年5月在佛山市中医院重症医学科接受肠内营养的60例成人患者,按注射方式不同分为A、B、C3组,每组20例。A组为采用注射器经胃管推注的传统推注组,B组为按正常就餐时间持续滴注组,C组使用新型恒温装置进行肠内营养支持,3组均接受常规治疗,并进行肠内营养支持。记录3组患者14d内每天肠内营养支持总量、相关并发症发生率,并比较治疗前后血浆白蛋白(Alb)、血红蛋白(Hb)水平。结果与A组和B组比较,C组每天肠内营养输注量最大(mL/d:1325±116比920±95、1013±86,均P〈0.05),腹泻、胃潴留、肺感染的发生率最低(腹泻:10%比30%、30%,胃潴留:15%比30%、25%,肺感染:25%比40%、35%,均P〈0.05)。14d后C组Alb水平最高(g/L:35.3±3.5比29.8±1.7、31.7±2.8,均P〈0.05)。结论新型肠内营养恒温装置的使用能提高Alb和Hb水平,减少相关并发症的发生,有一定临床应用价值。Objective To evaluate the effect of using a new enteral nutrition constant temperature device on coma patients after cardiopulmonary resuscitation and its influence on nutrition support complications. Methods Sixty aduh patients admitted to Department of Critical Care Medicine of Foshan Traditional Chinese Medicine Hospital to receive enteral support were enrolled, and they were divided into A, B, C groups by different injection modes, 20 cases in each group. Group A was given traditional injection via gastie tube with syringe, while group B was given continous injection according to normal meal time. The new constant temperature device was used in C group for enteral nutrition support. All the three groups were treated with conventional therapy. Over 14 days after admission, every patient's enteral nutrition total amount on each day, and the incidence of related complications in each group was recorded. After 14 days of treatment, the levels of plasma albumin (Alb) and blood hemoglobin were compared among the three groups. Results Compared with A group and B group, the mean nutrition amount administered was the highest (mUd: 1 325 ± 116 vs. 920 ± 95, 1 013±86, both P 〈 0.05), and the incidences of diarrhea, gastric retention, and pneumonia were the lowest (diarrhea: 10% vs. 30%, 30%, gastric retention: 15% vs. 30%, 25%, pneumonia: 25% vs. 40%, 35%, all P 〈 0.05) in C group; after 14 days, the level of plasma Alb was the highest in C group (g/L: 35.3 ± 3.5 vs. 29.8 ± 1.7, 31.7 ± 2.8, all P 〈 0.05). Conclusion The new enteral nutrition constant temperature device is of useful value in enteral nutrition support for coma patients after cardiopulmonary resuscitation, as it is capable of increasing the levels of Alb, Hb, and decreasing the incidence of related complications.
