带蒂睑板结膜瓣转位在眼睑重建中的应用

摘　　要：目的 评价利用剩余眼睑带蒂睑板结膜瓣转位在眼睑肿瘤切除术后眼睑较大范围全层缺损修补中的临床疗效.方法 对15例眼睑恶性肿瘤切除术后眼睑全层缺损在1/3至1/2的患者,采用剩余眼睑带蒂睑板结膜瓣转位替代眼睑后层,即睑板和睑结膜层;利用带眼轮匝肌滑行皮瓣或转移皮瓣修复眼睑前层,即皮肤肌肉层.结果 全部患者术后随访6～36个月（平均18个月）.无一例肿瘤复发,皮瓣全部成活,转位的带蒂睑板结膜瓣成活良好,眼睑外观及功能恢复满意.结论 剩余眼睑带蒂睑板结膜瓣转位可以重建眼睑恶性肿瘤切除术后1/3至1/2的的全层缺损,效果肯定.Objective To investigate the clinical effect of the remaining eyelid of Pedicled tarsal conjunctival flap transposition in the reconstruction of severe full-thickness eyelid defects after the surgery of malignant eyelid tumors.Methods Among the 15 patients （15 eyes） treated with full-thickness eyelid defects between 1/3 to 1/2 after the surgery of malignant eyelid tumors,the remaining eyelid of Pedicled tarsal conjunctival flap was grafted to replace the posterior layers of the eyelid （the tarsus and the conjunctiva） while periocular orbicularis muscular flap was utilized to reconstruct the anterior layers of the eyelid （the skin and muscular layer）.Results All patients were followed for 6 to 36 months,averaged 18 months.The flaps and Pedicled tarsal conjunctival flaps existed well in all cases.Satisfactory aesthetic and functional results were achieved,all patients with no recurrence.Conclusions The transposition of Pedicled tarsal conjunctival flap can repair full-thickness eyelid defects between 1/3 to 1/2 after the surgery of malignant eyelid tumors with definite and good outcome.
