血小板计数在急性髓系白血病化疗后的动态变化及临床意义

摘　　要：目的:分析血小板计数(PLT)的动态变化在急性髓系白血病(AML)患者诱导化疗后骨髓缓解中的意义。方法:选取明确诊断为AML且接受标准DA方案化疗的106例患者。根据化疗结束后第14天骨髓缓解情况分为缓解组与未缓解组,分别收集两组患者化疗结束后第10天、14天PLT计数资料,分析两组患者PLT计数在化疗后回升的时间及数量的动态变化与骨髓缓解情况的关系。结果:通过动态观察化疗后第10天、14天血小板计数变化,缓解组46例患者的PLT计数均高于未缓解组60例患者的PLT计数,差异有统计学意义(P<0.05);化疗后第10天,46例缓解组患者中,43例患者血小板计数≥20×10~9/L,其中25例>50×10~9/L;60例未缓解组患者中23例≥20×10~9/L,6例>50×10~9/L,差异有统计学意义(P<0.05)。结论:AML患者诱导化疗后血小板计数的动态变化与骨髓造血功能的恢复有一定的相关性,可作为临床上评估AML疗效简单易行的早期指标之一。Objective To analyze the significance of the dynamic changes of platelet count（ PLT） in the treatment of bone marrow in patients with acute myeloid leukemia（ AML） after induction chemotherapy. Method A retrospective analysis was performed in 106 patients with definite diagnosis of AML and chemotherapy with standard DA regimen. The patients were divided into remission group and non-remission group according to the bone marrow remission on the 14 th day after the chemotherapy. The PLT count data of the two groups were collected on the 10 th and 14 th day after the chemotherapy,respectively. The Relationship between dynamic changes of quantity and bone marrow remission. Results By dynamic observing the change of platelet counts on the 10 th day and 14 th day after chemotherapy,the PLT counts of 46 patients in remission group were significantly higher than those in 60 patients in non-remission patients（ P 0. 05）. On the 10 th day after chemotherapy,In 46 patients in remission group,the platelet count of 43 patients was ≥20 × 109/L,of which 25 cases were 50 × 109/L; in the 60 unrelated patients,23 cases were≥20 × 109/L,6 cases 50 × 109/L（ P 0. 05）. Conclusion The dynamic changes of platelet count after induction chemotherapy in AML patients have some correlation with the recovery of hematopoietic function of bone marrow,which can be used as one of the early indicators to evaluate the curative effect of AML clinically.
