摘  要:目的检测人类可溶性细胞程序性死亡受体-1/人类可溶性程序性死亡配体-1(s PD-1/s PD-L1)在外周血中表达情况,分析其意义,为食管鳞癌的诊断和治疗提供理论基础。方法选取正常对照组、食管鳞癌前病变组、食管鳞癌组3组,每组90例。应用酶联免疫吸附试验(ELISA)检测待测人群血清中s PD-1/s PD-L1的含量,统计学分析其与临床病理学特征的关系。结果(1)s PD-1/s PD-L1在正常对照、食管鳞癌癌前病变、食管鳞癌三组外周血中的含量分别为(601.66±92.52)/(173.95±22.30)pg/ml、(636.62±125.79)/(195.49±28.44)pg/ml、(649.22±101.72)/(219.05±27.98)pg/m l,比较差异有统计学意义(P<0.05)。(2)3组外周血s PD-1和s PD-L1相关性分析,均无相关性。(3)食管鳞癌患者外周血s PD-1的表达与食管鳞癌N分期密切相关(P<0.05),s PD-L1与肿瘤的最大径密切相关(P<0.05)。(4)s PD-L1正常组和高值组的PFS有统计学差异(P=0.026)。结论 s PD-1与食管鳞癌病变的发生有关,s PD-L1与食管鳞癌的发生、发展及预后有一定的关系。【Objective】To discuss the expression of human s PD-1/s PD-L1 in peripheral blood and its significance, and provide theoretical basis for the diagnosis and treatment of esophageal squamous carcinoma.【Methods】The research objects were divided into the normal group, esophageal precancerous lesion group and esophageal squamous carcinoma group, with 90 cases in each group. The contents of s PD-1/s PD-L1 were detected by ezyme-linked immunosorbent assay(ELISA). SPSS was used for statistical analysis.【Results】The contents of s PD-1/s PD-L1 in peripheral blood of the normal group, esophageal precancerous lesion group and esophageal squamous carcinoma group were respectively(601.66 ± 92.52)/(173.95 ± 22.30) pg/ml,(636.62 ± 125.79)/(195.49 ± 28.44) pg/ml,(649.22 ± 101.72)/(219.05 ± 27.98) pg/ml, and the differences were statistically significant(P〈0.05). No significant association was observed between s PD-1 and s PD-L1 expression in the normal group, esophageal precancerous lesion group and esophageal squamous carcinoma group. The expression of s PD-1 in peripheral blood of patients with esophageal squamous carcinoma was closely correlated to tumor N status(P〈0.05), and the expression of s PD-L1 was closely correlated to the maximum diameter of the tumor(P〈0.05). Progression-free survival(PFS) was significantly poorer in the s PD-L1 high value group than in the s PD-L1 normal group(P =0.026).【Conclusions】The s PD-1 is related to the occurrence of esophageal diseases, and the s PD-L1 is related to the genesis, progression and prognosis of esophageal squamous carcinoma.
