摘　　要：目的探讨MMPs在胃癌组织中的表达及意义.方法 SP免疫组化法检测胃癌组织、区域淋巴结、正常胃黏膜中MMPs的表达,同时分析其与胃癌患者人口学特征、疾病特征、病理学特征的关系.结果与正常胃黏膜比较,胃癌组织中MMP-2的阳性表达率明显升高,差异具有统计学意义(P<0.05).胃癌区域淋巴结组织中,有转移淋巴结组MMP-2的阳性表达率明显高于无转移淋巴结组,差异具有统计学意义(P<0.05);胃癌区域淋巴结组织中,MMP-2表达阳性组淋巴结转移率明显高于表达阴性组,差异具有统计学意义(P<0.05).胃癌组织中MMP-2的表达随着分化程度降低、浸润深度加深、淋巴结转移、临床分期增加而明显增强,差异具有统计学意义(P<0.05).胃癌组织中MMP-2表达阳性组患者5 a生存率明显低于表达阴性组,差异具有统计学意义(P<0.01);胃癌区域淋巴结组织中MMP-2表达阳性组患者5 a生存率明显低于表达阴性组,差异具有统计学意义(P<0.01).结论 MMP-2异常表达在胃癌的侵袭、转移中发挥关键作用;区域淋巴结组织中MMP-2表达异常与淋巴结转移及患者预后有关.Objective To investigate the expression and significance of MMPs in gastric cancer. Method The expression of MMPs in gastric cancer tissues,regional lymph nodes and normal gastric mucosa was detected by SP immunohistochemical method,and the relationship between the expression and the demographic characteristics,disease characteristics and pathological features of gastric cancer patients was analyzed. Results Compared with normal gastric mucosa,the positive expression rate of MMP-2 in gastric cancer tissue increased significantly,the difference was statistically significant（ P〈0.05）. In the lymph node tissue of gastric cancer,the positive expression rate of MMP-2 in lymph node group was significantly higher than that in non metastatic lymph node group,and the difference was statistically significant（ P〈0.05）. The lymph node metastasis rate in MMP-2 positive group was significantly higher than that in negative expression group（ P〈0.05）. The expression of MMP-2 in gastric carcinoma was significantly enhanced with the decrease of differentiation,depth of infiltration,metastasis of lymph nodes and clinical stages,and the difference was statistically significant（ P〈0.05）. The 5-year survival rate of patients with positive expression of MMP-2 in gastric cancer tissues was significantly lower than that in negative expression group（ P〈0.01）. The 5-year survival rate of patients with positive expression of MMP-2 in gastric lymph node tissues was significantly lower than that in negative expression group（ P〈0.01）.Conclusion Abnormal expression of MMP-2 plays a key role in invasion and metastasis of gastric cancer.Abnormal expression of MMP-2 in regional lymph nodes is related to lymph node metastasis and prognosis.
