VD化疗联合血液滤过治疗多发性骨髓瘤肾病的疗效分析

摘　　要：目的观察血液净化辅助VD化疗方案治疗骨髓瘤肾病的疗效。方法将本院收治的52例多发性骨髓瘤肾病患者随机分为两组,各26例。两组患者均应用VD化疗方案及常规治疗手段,观察组联合血液净化治疗。检测两组患者治疗前后血清肌酐、血尿酸、血钙和血清免疫球蛋白水平。结果治疗前,两组患者血清肌酐、血尿酸、血钙和血清免疫球蛋白水平无明显差异(P>0.05);治疗后,观察组患者血清肌酐、血钙和血尿酸水平显著低于对照组(P<0.05),而血清免疫球蛋白水平与对照组无明显差异(P>0.05)。观察组患者治疗总有效率显著高于对照组,差异有统计学意义(P<0.05)。结论对多发性骨髓瘤肾病患者采用血液净化辅助治疗可有效降低血清肌酐和血尿酸水平,提高治疗效果,有效改善肾功能,值得临床推广应用。Objective To observe the efficacy of adjuvant blood purification treatment for myeloma nephropathy patients with VD regi- men chemotherapy. Methods A total of 52 patients with multiple myeloma nephropathy admitted to our hospital were divided into two groups with 26 patients in each group. Patients in both groups got conventional treatment and chemotherapy of VD regimen. The observation group was added with blood purification treatment. We detected serum creatinine, serum uric acid, serum calcium, and serum immunoglob- ulin levels before and after treatment in both groups. Results There was no difference in serum creatinine, serum uric acid, serum calcium, and serum immunoglobulin levels between the observation group and control group before treatment （P 〉 0.05）. After treatment, the levels of serum creatinine, serum calcium and serum uric acid were lower in the observation group than in the control group （P 〈 0.05）, but there was no difference in serum immunoglobulin levels between the two groups （P 〉 0.05）. At the same time, the total effective rate of treatment was higher in observation group than in control group, and there was a statistically significant difference （P 〈 0.05）. Conclusion The combination of blood purification treatment and VD regimen chemotherapy for patients with multiple myeloma nephropathy can effectively reduce the se- rum creatinine and uric acid levels, improve the total effective rate, and effectively improve renal function.
