多西他赛、顺铂化疗联合同步调强放疗治疗食管癌术后复发的疗效分析

摘　　要：目的探讨多西他赛、顺铂化疗联合同步调强放疗治疗食管癌术后复发的疗效及对血清学指标的影响。方法收集本院收治的食管癌术后复发患者70例,随机分为对照组和观察组,各35例。对照组患者采用氟尿嘧啶、顺铂联合调强放疗,观察组患者采用多西他赛、顺铂化疗联合同步调强放疗。对比两组患者的近期疗效,血清肿瘤标志物、血清血管新生指标含量的差异。结果观察组患者的治疗总有效率显著高于对照组患者(P<0.05);观察组患者血清肿瘤标志物细胞角蛋白21-1片段(CYFRA21-1)、鳞状细胞癌抗原(SCC)、癌胚抗原(CEA)的含量显著低于对照组患者(P<0.05);观察组患者血清血管新生指标血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)、缺氧诱导因子1α(HIF-1α)的含量显著低于对照组患者(P<0.05)。结论食管癌术后复发患者接受多西他赛、顺铂化疗联合同步调强放疗治疗,可更有效地提高近期疗效并抑制肿瘤恶性程度。Objective To study the effect of doeetaxel and eisplatin chemotherapy combined with intensity-modulated radiotherapy on postoperative recurrence of esophageal cancer and its influence on serological indexes. Methods 70 eases of postoperative reeurrenee of esophageal cancer patients enrolled in our hospital were chosen as research subjects. All patients were randomly divided into control group and observation group, each with 35 eases. Patients in control group were treated with fluorouraeil and eisplatin chemotherapy combined with IMRT, and those in observation group were treated with doeetaxel and eisplatin chemotherapy combined with IMRT. The short-term effects of two groups were compared. The differences in serum tumor markers and serum angiogenesis indexes were observed. Results Total efficiency in observation group was significantly higher than in control group （P 〈 0.05）. The levels of serum tumor markers such as CY- FRA21-1, SCC, CEA were lower in observation group than in control group （P 〈 0.05）. In addition, the levels of angiogenesis indexes such as VEGF, MMP-9, HIF-1 a in observation group were significantly lower than in control group （P 〈 0.05）. Conclusion The treatment of do- eetaxel, eisplatin combined with concurrent intensity-modulated radiotherapy can effectively improve the short-term effects on esophageal cancer patients with postoperative reeurrence, and inhibit the malignancy of tumor.
