摘　　要：目的分析小儿肝母细胞瘤的临床特点及疗效。方法回顾性分析郑州大学第一附属医院小儿外科2012年1月至2017年1月收治的经病理证实的30例肝母细胞瘤的临床病理资料,对其诊断、治疗经过及疗效进行分析和总结。结果 30例肝母细胞瘤患儿,19例无病生存,11例死亡。单因素分析结果显示,手术完整切除(P=0.001)、确诊时无肝外转移(P=0.000)、新辅助化疗后延期手术(P=0.000)、术后肿瘤无复发(P=0.000)患儿预后较好。多因素分析结果显示,手术完整切除可作为判断患儿预后的独立因素。结论术前化疗后,肿瘤可完全切除的肝母细胞瘤患儿长期生存率较高,化疗后手术仍无法切除或治疗后复发患儿预后较差。Objective To analyze the clinical characteristics and therapeutic efficacy of children with hepatoblastoma.Methods The clinical and pathological data of 30 cases of hepatoblastoma confirmed by pathology between January of 2012 and January of 2017 in pediatric surgery of the First Affiliated Hospital of Zhengzhou University were reviewed. The treatment process and curative effect were analyzed and summarized. Results There were 30 cases of hepatoblastoma in this group. By the end of follow-up,there were 19 cases of disease-free survival and 11 cases of death. The single factor analysis showed that children with complete resection of the operation（ P = 0. 001）,at the time of diagnosis of extrahepatic metastasis（ P = 0. 000）,delayed surgery after neoadjuvant chemotherapy（ P = 0. 000） and no recurrence of the tumor（ P = 0. 000） had better prognosis. Multivariate analysis suggested that complete resection could be an independent prognostic factor. Conclusion After preoperative chemotherapy,the long-term survival rate of children with completely resected hepatoblastoma is high,and the prognosis of children with unresectable tumor or relapsed after treatment is poor.
