牙隐裂伴牙髓炎患者根管治疗后全瓷冠修复和金属烤瓷冠修复的效果比较

摘　　要：目的比较牙隐裂伴牙髓炎患者根管治疗后全瓷冠修复和金属烤瓷冠修复的效果。方法选取2015年6月至2016年8月首都医科大学附属北京安贞医院收治的牙隐裂伴牙髓炎患者76例。根据患者根管治疗后修复方式分为对照组(金属烤瓷冠修复,39例)和观察组(全瓷冠修复,37例)。冠修复治疗后随访12个月,比较2组患者冠修复效果和SF-36量表生活质量评分。结果观察组失访1例,有效研究36例;对照组治疗期间退出研究1例,失访1例,有效研究37例。观察组冠修复治疗总有效率明显高于对照组[97.2%(35/36)比78.4%(29/37)](P=0.036)。冠修复后,观察组菌斑指数、牙龈出血指数、牙周探诊深度均明显低于对照组[(1.0±0.6)比(1.5±0.8)、0.11(0.02,0.37)比0.98(0.03,1.95)、(2.0±0.6)mm比(2.5±0.9)mm](均P<0.05)。观察组生理职能、生理功能、社会职能、健康状况评分均明显高于对照组[(16.4±2.1)分比(13.1±1.9)分、(17.1±1.8)分比(15.4±2.0)分、(18.3±1.1)分比(17.5±1.7)分、(17.5±1.6)分比(16.0±1.9)分](均P<0.05)。结论牙隐裂伴牙髓炎患者根管治疗后全瓷冠修复可明显提高临床疗效和生活质量,其治疗效果优于金属烤瓷冠修复。Objective To compare the effects of all-ceramic crown and metal-ceramic crown after root canal therapy on patients with cracked-teeth and pulpitis.Methods A total of 76 cases of cracked-tooth with pulpitis were enrolled from June 2015 to August 2016 in Beijing Anzhen Hospital,Capital Medical University.They were divided into control group（metal-ceramic crown,n =39） and observation group（all-ceramic crown,n =37）.Clinical effect of crown restoration and quality of life assessed by the SF-36 scale were analyzed 12 months after treatment.Results There was 1 loss of follow-up in the observation group.There was 1 withdrawl and 1 loss of follow-up in the control group.Effective rate of crown restoration in the observation group was significantly higher than that in the control group[97.2% （35/36） vs 78.4% （29/37）] （P =0.036）.After treatment,plaque index,gingival bleeding index and periodontal probing depth in the observation group were significantly lower than those in the control group [（1.0 ± 0.6） vs （1.5 ± 0.8）,0.11 （0.02,0.37） vs 0.98 （0.03,1.95）,（2.0 ± 0.6） mm vs （2.5 ± 0.9） mm] （P 〈 0.05）.Scores of physical role,physiological function,social role and health condition in the observation group were significantly higher than those in the control group[（16.4 ± 2.1） vs （13.1 ± 1.9）,（17.1±1.8） vs （15.4±2.0）,（18.3±1.1） vs （17.5±1.7）,（17.5±1.6） vs （16.0±1.9）]（P〈0.05）.Conclusion All-ceramic crown can more effectively improve the clinical effect and quality of life than metal-ceramic crown in cracked-teeth with pulpitis after root canal therapy.
