摘 要:目的探讨血清糖类抗原125(CA125)、鳞状细胞癌抗原(SCCAg)、胰岛素样生长因子-Ⅱ(IGF-Ⅱ)及细胞角蛋白19片段(CYFRA21-1)联合检测在宫颈癌早期诊断中的价值。方法选取2016年6月至2017年6月间我院收治的65例宫颈癌患者作为观察组,并选取同期收治的50例宫颈良性病变者作为疾病对照组以及50例体检合格的健康妇女作为健康对照组。对比三组血清CA125、SCCAg、IGF-Ⅱ、CYFRA21-1水平及阳性检出率,并观察上述指标单独检测及联合检测对宫颈癌的诊断效率。结果观察组患者血清CA125、SCCAg、IGF-Ⅱ及CYFRA21-1水平均显著高于两对照组(P<0.05);不同临床分期宫颈癌患者的血清CA125、SCCAg、IGF-Ⅱ及CYFRA21-1水平比较差异具有统计学意义(P<0.05);观察组CA125、SCCAg、IGF-Ⅱ及CYFRA21-1检测的阳性率均明显高于两对照组(P<0.05)。此外,血清CA125、SCCAg、IGF-Ⅱ及CYFRA21-1四者联合检测的灵敏度相比于各指标单独检测均显著升高(P<0.05);而联合检测的特异性相比于单独检测均有所降低,且相比于SCCAg、CYFRA21-1差异具有统计学意义(P<0.05);同时联合检测的准确度相比于单独检测均有所升高,但差异无统计学意义(P>0.05)。结论血清CA125、SCCAg、IGF-Ⅱ及CYFRA21-1用于宫颈癌的早期诊断,表现出一定的临床价值,而四种标志物联合检测显著提高了诊断的灵敏度,也提高了诊断的准确度,一定程度地避免了漏诊及误诊,具有更大的临床应用价值。Objective To explore the dianostic value of the serum CA125, SCCAg ,IGF- Ⅱ and CYFRA21-1 in patients with cervical carcinoma. Methods A total of 65 patients with cervical carcinoma treated in our hospital from June 2016 to June 2017 were selected as observation group,45 patients with benign cervical lesion treated in the same period were slected as disease control group, and 45 normal volunteers were in health control group. The level and postive rate of CA125 ,SCCAg,IGF- Ⅱ and CYFRA21-1 were compared between the three groups, and the diagnostic efficency of CA125, SCCAg, IGF-Ⅱ and CYFRA21-1 for cervical carcinoma were observed. Results The serum CA125, SCCAg, IGF- Ⅱ and CYFRA21-1 level were significantly higher in the observation group than those in other two control groups ( P 〈 0.05 ). The level of CA125, SCCAg ,IGF- Ⅱand CYFRA21-1 in different clinical stages were statistically significant( P 〈 0.05 ). The positive rate of CA125 ,SCCAg,IGF-Ⅱ and CYFRA21-1 were significantly higher in the observation group than those in the two control groups ( P 〈 0. 05 ). In addition, the sensitivity of joint detection of CA125, SCCAg ,IGF- Ⅱ and CYFRA21-1 was significantly higher than separate detection (P 〈 0.05 ). The specificity of joint detection was lower than separate detection, and compared with SCCAg, CYFRA21 - 1, and the difference were statistically significant( P 〈 0.05 ). But the accuracy of the joint detection was just slightlyhigher than separate detection ( P 〉 0.05 ). Conclusion The CA125, SCCAg, IGF- Ⅱ and CYFRA21-1 have good diagnosic values for the cervical carcinoma. The joint detection of CA125, SCCAg, IGF-Ⅱ and CYFRA21-1 can significantly increase the sensitivity of detection, slightly enhance the accuracy of diagnosis, avoid missed diagnosis and misdiagnosis, and have a high value of clinical application.
