摘 要:目的探讨检测血常规、血清铁蛋白在缺铁性贫血(IDA)及合并地中海贫血患儿补铁治疗中的临床意义。方法将2015年2月-2017年8月本院儿科门诊随访治疗的30例地中海贫血伴缺铁性贫血患儿设为观察组,同期治疗的30例单纯缺铁性贫血患儿设为对照组,两组患儿均适当予以补充铁剂,并监测比较两组患儿治疗前、治疗后(7 d、20 d、30 d)的血常规及血清铁代谢各项指标变化,并根据其变化情况指导临床补铁治疗。结果两组患儿治疗前的各项血常规和血清铁代谢指标相比差异无统计学意义(P>0.05),但两组患儿治疗后7 d、治疗后20 d和治疗后30 d的各项指标均明显高于治疗前(P<0.05),但两组患儿治疗后不同时间段的Hb、MCH和MCHC水平相比差异无统计学意义(P>0.05);观察组患儿治疗后的MCV水平显著低于对照组(P<0.05);观察组患儿治疗后7 d、治疗后20 d和治疗后30 d的血铁清和SF水平明显高于对照,差异有统计学意义(P<0.05)。结论地中海贫血伴缺铁性贫血患儿与单纯缺铁性贫血患儿均在补铁治疗后症状改善明显,但地中海贫血患儿对补铁治疗更为敏感,需严密监测患儿血常规及血清铁代谢各项指标,适时停止补铁,并进行合理膳食,达到综合治疗。Objective To investigate the clinical significance of detecting blood routine and serum ferritin in patients with iron deficiency anemia and iron supplementation in children with thalassemia. Methods A total of 30 children with thalassemia and iron deficiency anemia who underwent pediatric outpatient follow-up from February 2015 to August 2017 were enrolled in this study. 30 children with iron deficiency anemia were enrolled as the control group, two groups of children were appropriate to be iron supplementation, and monitoring the two groups of children before and after treatment(7 d, 20 d, 30 d)blood and serum iron metabolism indicators, and according to changes in the direction of clinical iron treatment. Results There was no significant difference in blood routine and serum iron metabolism between the two groups before treatment(P〉0.05). However, all the indexes in the two groups at 7 days, 20 days and 30 days after treatment(P〈0.05). However, there was no significant difference in the levels of Hb, MCH and MCHC between the two groups at different time points after treatment(P〉0.05). The MCV levels in the treatment group(P〈0.05). The levels of serum iron and serum SF in the observation group at 7 days, 20 days and 30 days after treatment were significantly higher than that of in the control group(P〈0.05). Conclusion Children with thalassemia and iron deficiency anemia, children with iron deficiency anemia improved significantly after treatment with iron supplementation. However, children with thalassemia were more sensitive to iron supplementation, and blood and serum Iron metabolism indicators, timely stop iron supplements, and a reasonable diet to achieve comprehensive treatment.
