CT在术前结肠癌及术后结肠癌复发诊断鉴别中的应用价值

摘　　要：目的:总结探讨CT在术前结肠癌及术后结肠癌复发诊断鉴别中的应用价值。方法:选择2015年1月~2017年9月期间我院收治或取得病理标本的35例结肠癌患者为研究对象,分析对比其术前CT诊断结论与病理诊断结论的符合率以及术后结肠癌复发患者的CT诊断结论与病理诊断结论的符合率,总结CT在结肠癌患者术前及术后复发诊断中的应用价值。结果:以病理切片诊断结果为金标准,CT在术前结肠癌患者诊断中的准确率为82.86%(29/35),其中T分期诊断准确率为84.21%(16/19)、N分期诊断准确率为80.00%(8/10)、M分期诊断准确率为83.33%(5/6);CT在术后结肠癌复发患者诊断中的准确率为83.33%(15/18),其中T分期诊断准确率为88.89%(8/9)、N分期诊断准确率为83.33%(5/6)、M分期诊断准确率为66.67%(2/3)。结论:CT检查不论是在结肠癌患者手术前还是手术后复发诊断中都具有极高的应用价值,可清楚显示病变情况,临床上值得推广使用。Objective： to summarize the value of CT in the differential diagnosis of colon cancer and recurrence of colon cancer after operation. Methods： thirty-five colon cancer patients admitted to our hospital from January 2015 to September 2017 were selected as subjects. The coincidence rate of preoperative CT findings with pathological findings and the coincidence rate of CT findings with pathological findings were compared. The application value of CT in the diagnosis of recurrence of colon cancer before and after operation was summarized. Results： the accuracy of CT in the preoperative diagnosis of colon cancer was 82. 86 %（29/35）,of which the accuracy of T staging was 84. 21 %（16/19）,and the accuracy of N staging was 80. 00 %（8/10）,the accuracy of M staging was 83. 33 %（5/6）. The accuracy of CT in the diagnosis of postoperative recurrence of colon cancer was 83. 33 %（15/18）,of which the accuracy of T staging was 88. 89 %（8/9）,the accuracy rate of N staging was 83. 33 %（5/6） and the accuracy rate of M staging was 66. 67 %（2/3）.Conclusion： CT examination is of great value in the diagnosis of recurrence of colon cancer before or after operation,and it can show the pathological changes clear. So it is worth popularizing in clinical practice.
