腹腔镜与开放性右半结肠癌根治术治疗结肠癌患者的疗效比较

摘　　要：目的探讨腹腔镜与开放性右半结肠癌根治术治疗结肠癌患者的临床疗效。方法选取2016年1月至2017年12月间广东省东莞东华医院收治的78例右半结肠癌患者,根据治疗方法不同分为观察组41例与对照组37例。观察组患者采用腹腔镜下右半结肠癌根治术治疗,对照组患者采用开放性右半结肠癌根治术治疗,比较两组患者手术指标、胃肠功能恢复情况和并发症发生率。结果观察组患者手术时间和术中出血量均少于对照组,淋巴结清扫数目多于对照组,切除标本长度小于对照组,差异均有统计学意义(均P<0.05)。观察组患者开始进食时间、首次肛门排气时间、肠鸣音恢复时间和首次排便时间均短于对照组,差异均有统计学意义(均P<0.05)。观察组患者并发症发生率为9.8%,低于对照组的32.4%,差异有统计学意义(P<0.05)。结论腹腔镜右半结肠癌根治术能减少右半结肠癌患者术中出血量,彻底清除淋巴结,保护胃肠功能,降低并发症发生率。Objective To investigate the curative efficacy between laparoscopic and open right hemicolectomy in the treatment of colon cancer.Methods Seventy-eight patients with right colon cancer treated at Dongguan Donghua Hospital from January 2016 to December 2017 were selected and divided into an observation group（41 patients） and a control group（37 patients） according to the treatment methods.The observation group underwent laparoscopic right hemicolectomy and the control group underwent open right hemicolectomy.General conditions,gastrointestinal function recovery and incidence of complications were compared between the two groups.Results The operation time,intraoperative blood lossl and the length of specimens resected in the observation group were higher than in the control group（all P〈0.05）.The time to eat,time to anal exhaust,the return of bowel sounds,and the time to defecate in the observation group were shorter than in the control group（all P〈0.05）.The incidence of complications in the observation group was 9.8%,which was significantly lower than 32.4% of the control group（P〈0.05）.Conclusion Laparoscopic right hemicolectomy can significantly reduce intraoperative blood loss,completely remove lymph nodes,protect gastrointestinal function and reduce the incidence of complications in patients with right colon cancer.
