腔内隔绝术治疗瘤颈扭曲大于60度腹主动脉瘤的效果

摘　　要：目的探讨腔内隔绝术(EVAR)治疗瘤颈扭曲大于60°腹主动脉瘤的效果。方法选取2016年4月至2017年12月间陕西省人民医院收治的66例瘤颈扭曲大于60°腹主动脉瘤患者进行回顾性分析,将其中采用EVAR治疗的33例患者纳入观察组,采用常规手术方法治疗的33例患者纳入对照组。比较两组患者的肾功能变化、血压、心率变化、手术时间、术中输血量、术后下床活动时间、禁食时间、住院时间及副反应量表(TESS)评分。结果术后观察组患者血尿素氮以及血肌酐均低于对照组患者,差异均有统计学意义(均P<0.05)。观察组患者的心率、舒张压和收缩压变化均优于对照组患者,差异均有统计学意义(均P<0.05)。观察组患者手术时间、术中输血量、术后下床活动时间、禁食时间及住院时间均低于对照组患者,差异均有统计学意义(均P<0.05)。观察组不良反应发生率为6.0%,低于对照组的27.3%,差异有统计学意义(P<0.05)。结论在治疗瘤颈扭曲大于60°腹主动脉瘤的过程中,EVER的治疗效果比较明显,应当加强推广应用。Objective To explore the efficacy of endovascular aneurysm repair（or endovascular aortic repair）（EVAR） in the treatment of abdominal aortic aneurysm（AAA） with tumor neck angle of more than 60 degrees.Method From April 2016 to December 2017,66 AAA patients with tumor neck angle of more than 60 degrees who were treated at Shaanxi People＇s Hospital were selected and retrospectively analyzed.Among them,33 patients who received EVAR therapy were included in the observation group and the rest 33 patients who received routine therapy were included in the control group.Renal function,blood pressure,heart rate changes,operation time,intraoperative blood transfusion,postoperative out of bed activity time,fasting time,length of hospital stay and treatment emergent symptom scale（TESS） score were compared between the two groups.Results After the operation,the blood BUN and blood creatinine were significantly better in the observation group than in the control group,（all P〈0.05）.Heart rate and changes in diastolic and systolic blood pressure were significantly better in the observation group than in the control group（all P〈0.05）.The operation time,intraoperative blood transfusion,postoperative out of bed activity time,fasting time,length of hospital stay were lower in the observation group than in the control group（all P〈0.05）.The incidence of adverse reactions was 6.0% for the observation group which was lower than 27.3% of the control group（P〈0.05）.Conclusion In the treatment of abdominal aortic aneurysm with tumor neck angle of more than 60 degrees,the efficacy of endovascular aneurysm repair is obvious,which should be popularized.
