姑息护理对老年晚期恶性肿瘤终末期患者的影响

摘　　要：目的探讨姑息护理对老年晚期恶性肿瘤终末期患者的影响。方法选取2016年1月至2017年6月间四川省简阳市人民医院收治的68例老年晚期恶性肿瘤终末期患者进行回顾性分析。将采用常规护理及姑息护理干预的38例患者纳入观察组,采用常规护理的30例患者纳入对照组。比较两组患者护理干预后的汉密尔顿焦虑量表评分、汉密尔顿抑郁量表评分、精神面貌、死亡恐惧症和家属心态等指标,并比较两组患者护理前后的生活质量。结果观察组护理后每个疗程结束时的焦虑评分和抑郁评分均低于对照组患者,差异均有统计学意义(均P<0.05)。观察组患者护理后的精神面貌、患者心态、死亡恐惧症和家属心态均优于对照组患者,且观察组患者在健康和卫生、安全领域和心理领域三个方面护理后的生活质量评分均高于对照组患者,差异均有统计学意义(均P<0.05)。结论姑息护理能提高老年晚期恶性肿瘤终末期患者的生活质量,缓解患者负性心理,建议推广和使用。Objective To explore the effect of palliative nursing on the end stage malignant tumors in the elderly patients.Methods From January 2016 to June 2017,68 patients with advanced malignant tumors treated at Jianyang People＇s Hospital were selected and retrospectively analyzed.The observation group received palliative nursing（38 patients） besides routine nursing and the control group（30 patients）was only given routine nursing.Hamilton anxiety scale（HAMA） and Hamilton depression scale（HAMD）scoles,mental outlook,death phobia,family members＇ attitude and quality of life were compared between the two groups before and after nursing.Results The HAMA and HAMD at the end of each nursing course,were lower in the intervention group than in the control group（all P〈0.05）.After nursing care,in the observation group,mental outlook,patients＇ mentality,death phobia and family members＇ psychology were significantly superior to those of the control group,The observation group was superior to the control group in the fields of quality of life including health,hygiene and safety（all P〈0.05）.Conclusion Palliative nursing can effectively improve the quality of life symptoms and relieve adverse moods,in the elderly with advanced malignant tumor.It is suggested that the palliative nursing should be popularized and used.
