摘　　要：目的探讨三维适形放疗与肝动脉栓塞化疗联合应用于原发性肝癌门静脉癌栓中的效果。方法选择2014年1月—2016年12月我院收治的原发性肝癌门静脉癌栓患者80例,以随机数字表将其分为对照组与研究组,各40例。对照组采用肝动脉栓塞化疗,研究组在对照组的基础上联合三维适形放疗。对比两组治疗3个月时的临床疗效,以及1年内的生存率。结果研究组治疗的总有效率为77.50%,高于对照组55.00%(P<0.05)。研究组1年内的生存率82.50%,高于对照组57.50%(P<0.05)。结论三维适形放疗与肝动脉栓塞化疗联合应用于原发性肝癌门静脉癌栓中疗效确切,可以有效提高患者的生存率。Objective To investigate the effect of three-dimensional conformal radiotherapy combined with hepatic artery embolization chemotherapy on portal vein tumor thrombi in primary liver cancer. Methods A total of 80 patients with portal vein tumor emboli from January 2014 to December 2016 in our hospital were selected and randomly divided into a control group and a study group with 40 patients in each group. The control group was given hepatic artery embolization chemotherapy. The study group was combined with three-dimensional conformal radiotherapy on the basis of the control group. The clinical efficacy of the two groups at the 3 months of treatment and the survival rate within 1 year were compared. Results The total effective rate of the study group was 77.50%, which was higher than that in the control group of 55.00%（P 〈 0.05）. The survival rate of the study group within one year was 82.50%, which was higher than that of the control group of 57.50%（P 〈 0.05）. Conclusion Three-dimensional conformal radiotherapy combined with hepatic artery embolization and chemotherapy is effective in the application of portal vein tumor thrombosis in primary hepatic carcinoma. It can effectively improve the survival rate of patients.
