血尿酸水平与2型糖尿病周围神经病变的相关性研究

摘　　要：目的研究2型糖尿病患者血尿酸水平与糖尿病周围神经病变的相关性。方法选取2017-01—2018-03在郑州大学第二附属医院内分泌科就诊的2型糖尿病患者306例。分为2型糖尿病(T2DM)组116例,2型糖尿病周围神经病变(DPN)190例;DPN组根据神经肌电图结果分为轻度、中度、重度亚组;按四分位法将SUA分为4组;采用Logistic回归分析方法分析血尿酸(SUA)与DPN的关系。结果 DPN组SUA、血肌酐(SCR)、视黄醇结合蛋白(RBP)、空腹C肽(FC-P)水平明显高于T2DM组,差异有统计学意义(P<0.01);SUA与DPN严重程度相关,差异有统计学意义(P<0.05);Logistic回归分析显示,SUA水平是DPN的独立危险因素,OR值1.004(95%CI:1.000~1.008)(P<0.05)。结论 2型糖尿病患者中,SUA是DPN的危险因素,其水平与DPN严重程度正相关。Objective To investigate the correlation between serum uric acid（SUA）level and diabetic peripheral neuropathy（DPN）.Methods A total of 306 patients with type 2 diabetes mellitusfrom January 2017 to March 2018 in the Endocrinology Department of the second affiliated Hospital of Zhengzhou Universitywere enrolled in this study.All patients were divided into two groups：type A（T2 DM）（n=115）and type B（DPN）（n=191）.The B group was further divided into three subgroups（mild,moderate and severe group）according to the results of electromyography;The SUA was divided into 4 groups according to quartile method;The binary logistic regression was used for statistical analysis of risk factors for DPN.Results SUA,SCR,RBP and FC-P were significantly higher in B group（321.99±86.386）umol/L than in A group（287.32 ±75.530）umol/L（P〈0.01）;The SUA was positively correlated with the severity of DPN（P〈0.05）;Logistic regression analysis showed that SUA was an independent risk factor for DPN.OR：1.004（95% CI：1.000-1.008）（P〈0.05）.Conclusion SUA is positively correlated with the severity of DPN.SUA level is a risk factor for the development of DPN with T2 DM.
