摘  要:目的 探讨ATRX和P53基因突变在中国人弥漫性胶质瘤分类中的指导作用.方法 收集2016年至2017年各级星形细胞瘤(IDH突变型或IDH野生型)、各级少突胶质细胞瘤89例.采用免疫组织化学染色法检测ATRX蛋白失表达及突变P53蛋白过表达,分析二者在不同类型弥漫性胶质瘤中的表达特点.结果 星形细胞瘤(IDH突变型)ATRX失表达比(17/24)高于少突胶质细胞瘤(3/16),P〈0.01;星形细胞瘤(IDH突变型)P53过表达比(15/24)高于少突胶质细胞瘤(1/16),P〈0.01;星形细胞瘤(IDH突变型)ATRX失表达比为71%(17/24),高于星形细胞瘤(IDH野生型)的41%(20/49),P〈0.05.结论 ATRX和P53基因突变是星形细胞瘤(IDH突变型)的分子遗传特征之一,可以辅助用于星形细胞瘤的鉴别诊断.Objective To investigate the role of ATRX and P53 gene mutations in the classification of diffuse glioma in Chinese. Methods A total of 89 cases of diffuse astrocytoma ( IDH mutation) or diffuse astrocytoma (IDH wild type) and all levels of oligodendroglioma from 2016 to 2017 were col- lected, and detected the expression of ATRX protein and over expression of P53 protein by immunohisto- chemistry, and analyzed their expression in different types of diffuse gliomas. Results The ratio of AT- RX loss expression in diffuse astroeytoma (IDH mutation) (17/24) was higher than that in oligodendro- gliomas ( 3/16 ), P 〈 0. 01 ; the ratio of P53 over expression in diffuse astrocytoma ( IDH mutation ) (15/24) was higher than that in oligodendrogliomas (1/16), P 〈 0.01; the ratio of ATRX loss expression in diffuse astrocytoma (IDH mutation) (71%, 17/24) was higher than that in diffuse astroeytoma ( IDH wild) (41%, 20/49 ), P 〈 0.05. Conclusions ATRX and P53 mutation is one of the molecular genetic characteristics of diffuse astrocytoma ( IDH mutation), which may be contributed to diagnose diffuse astrocytoma.
