溃结平方联合西药治疗轻中度溃疡性结肠炎活动期患者100例临床观察

摘　　要：目的探讨溃结平方联合西药治疗轻中度溃疡性结肠炎活动期患者的临床疗效。方法 200例轻中度溃疡性结肠炎活动期患者按照是否愿意接受中药治疗按1∶1比例匹配分为观察组和对照组各100例。对照组采用常规西医治疗,观察组在对照组的基础上口服中药溃结平方,每日1剂。治疗前后进行结肠黏膜病变Baron内镜评分、主要症状评分(包括腹泻、腹痛、腹胀、脓血便、里急后重、肛门灼热),检测炎症及免疫学指标[包括血清C反应蛋白(CRP)、白细胞介素10(IL-10)、免疫球蛋白G(Ig G)、补体C3],治疗后判断结肠黏膜病变疗效及主要症状疗效,观察试验期间不良反应。结果两组患者治疗后Baron内镜评分、主要症状总分及血清CRP、Ig G、补体C3水平均较本组治疗前降低,IL-10水平升高(P<0.05);且观察组治疗后Baron内镜评分、主要症状总分及血清CRP、Ig G、补体C3水平低于对照组治疗后,IL-10水平高于对照组治疗后(P<0.05)。观察组治疗后结肠黏膜病变疗效总有效率、主要症状疗效均为96.0%,对照组分别为76.0%、80.0%,观察组均优于对照组(P<0.05)。治疗过程中观察组不良反应发生率为4.0%,对照组为11.0%(P<0.01)。结论溃结平方联合西药治疗轻中度溃疡性结肠炎活动期患者临床效果优于单纯西药,且安全性好。Objective To study the clinical efficacy of integrative Kui Jieping Formula（溃结平方） and western medicine in the treatment of patients with mild and moderate ulcerative colitis at the active stage. Methods A total of 200 patients with mild and moderate ulcerative colitis at the active stage were divided into observation group（100 cases） and control group（100 cases） according to their willingness to accept traditional Chinese medicine treatment.The control group was given conventional western medicine therapy,and the observation group was added Kui Jieping Formula by oral administration on the basis of control group,one formula each day. Baron endoscope score of ulcerative colitis,main symptom scores including diarrhea,abdominal pain,abdominal distension,bloody purulent stool,tenesmus and burning sensation in anus were evaluated before and after treatment. The inflammation and immunological index including serum c-reactive protein（CRP）,interleukin 10（IL-10）,Immunoglobulin G（IgG） and complement C3 were detected. The efficacy on ulcerative colitis and main syndromes after the treatment was determined. Adverse reactions during the study were observed. Results Baron endoscope score,main symptom score and serum CRP,IL-10 and complement C3 in both groups were declined after the treatment,and the level of IL-10 was increased（P〈0. 05）. Baron endoscope score,main symptom score,and the level of CRP,Ig G,complement C3 in serum in observation group was lower than the control group after the treatment,while the level of IL-10 was higher than the control group（P〈0. 05）. The total effective rate in treating ulcerative colitis and main symptoms of the observation group were all 96. 0%,while 76. 0% 、80. 0% in control group. The results in observation group were all superior to the control group（P〈0. 05）.
