摘 要:目的探讨急性白血病患者血清肿瘤相关物质(TAM)与血液及骨髓其它检验指标相关性,分析TAM在急性白血病中的应用价值。方法选择2013年6月~2018年3月赣州市立医院和赣南医学院第一附属医院血液科急性白血病患者320人次,急性白血病患者分为两组,其中一组为160名急性淋巴细胞白血病,另一组为160名急性髓系细胞白血病。另外选择正常体检患者160人次为正常对照组。分别观察急性白血病患者初诊时、化疗达缓解后与对照组各项指标,包括各组白细胞计数、红细胞计数、血红蛋白浓度、血小板计数以及各组Wilms肿瘤基因(Wilms'tumor gene,WT1)阳性表达率、骨髓原始细胞比例与肿瘤相关物质(TAM)水平,探讨各项指标之间的相关性。结果实验各组白细胞计数、红细胞计数、血红蛋白浓度、血小板计数、各组WT1阳性表达率、骨髓原始细胞比例、TAM含量在初诊时、化疗达缓解后比较有统计学意义(P<0.05);AML缓解组WT1阳性率31.25%较初诊组WT1阳性率92.5%,TAM阳性率5.625%较初诊组TAM阳性率66.25%均是下降明显;ALL缓解组WT1阳性率21.875%较初诊组WT1阳性率52.5%,TAM阳性率8.75%较初诊组TAM阳性率46.25%同样下降较多。TAM与上述血液常规指标以及骨髓相关指标之间存在一定的相关性。结论在急性白血病中TAM表达水平、阳性率在初诊组、缓解组和WT1表达水平、阳性率具有相当的一致性。TAM可以与WT1检测、血细胞分析、骨髓原始细胞比例等一起作为监测急性白血病疗效判断、监测病情复发的指标。Objective To study correlation among the serum tumor associated material (TAM) in acute leukemia examination and blood routine substance as well as other routine indexes, and to analyze if there is any early diagnosis value with TAM. Methods To choose 320 cases of acute leu- kemia patients from the Ganzhou Municipal Hospital and the First Affiliated Hospital of Gannan Medical University for research and to select 160 patients of normal physical examination as the normal group from June 2013 to March 2018, to observe blood routine indexes between groups by white blood cell average, red blood cells average, hemoglobin concentration range, platelet average, WT1 positive expression rate, primitive bone marrow cells average and TAM content in patients before chemotherapy and after remission, analyse the relationship between the TAM and other test items. Results There were statistically significant differences in white blood cell count, RBC count, hemoglobin concentration, platelet count, positive WT1 expression rate, bone marrow primitive cell ratio, TAM content at the initial diagnosis and after chemotherapy (P〈0.05). The positive rate of WT1 in the AML mitigation group was 31.25%, which was 92.5% compared with that in the initial treatment group, and the positive rate of TAM was 5.625%, which was significantly lower than that in the initial treatment group. The positive rate of WT1 in the ALL remission group was 21.875%, which was 52.5% higher than that in the initial treatment group, while the positive rate of TAM was 8.75%, which was 46.25% lower than that in the initial treatment group. There is a correlation between TAM and the blood routine indexes above and the related indicators of the bone marrow. Conclusion The expression level and positive rate of TAM were similar with WT1 and the blood routine indexes above in patients with acute leukemia before chemotherapy and after remission.
