联合检测CA125、CA153及CEA在乳腺癌患者中的应用价值及临床意义

摘　　要：目的探讨联合检测糖类抗原153(CA153)、糖类抗原125(CA125)及癌胚抗原(CEA)在乳腺癌患者中的应用价值及临床意义。方法回顾性分析了2014年1月至2016年1月就诊患者资料,分为观察组、良性对照组和健康对照组,比较三组CA125、CA153及CEA的检测水平差异;进一步分析单独检测及联合检测上述指标的阳性率、灵敏度、特异度及准确度之间的差异;根据乳腺癌组不同的分期,探讨CA125、CA153及CEA检测水平和分期之间的关系。结果观察组CA125、CA153、CEA水平明显高于良性对照组和健康对照组,差异具有统计学意义(P<0.05);联合检测上述指标的阳性率明显高于单独检测;联合检测的灵敏度及准确度等指标较单独检测有明显的提高;随着乳腺癌分期的进展,CA125、CA153、CEA水平也相应的升高。结论与单独检测相比,联合检测CA125、CA153、CEA具有较高的阳性率、灵敏度及准确度,可以作为乳腺癌患者的治疗效果的判断及术后随访的指标,具有一定的临床应用价值。Objective To evaluate the application value and clinical significance of combined detection of serum carbohydrate antigen 153（CA153）, carbohydrate antigen 125（CA125） and carcinoembryonic antigen（CEA） in patients with breast cancer. Methods The patients who were detected with serum CA125, CA153 and CEA from January 2014 to January2016 were retrospectively reviewed. All the patients were divided into observation group, benign control group and healthy control group. The detection levels of serum CA125, CA153 and CEA were compared among the three groups. The differences of positive rate, sensitivity, specificity and accuracy were further analyzed between single detection and combined detection. The relationship between the detection levels of serum CA125, CA153 and CEA the stage was discussed according to the different stage of the breast cancer group. Results The levels of CA125, CA153 and CEA in the observation group were significantly higher than those in the benign control group and healthy control group, the differences were statistically significant（P〈0.05）. The positive rates of combined detection were significantly higher than those of single deteltion. The sensitivity and accuracy of combined detection significantly improved compared with single detection. With the progress of breast cancer stage, the levels of serum CA125, CA153, CEA increased accordingly. Conclusion Compared with single detection, there are higher positive rate, sensitivity and accuracy of combined detection for CA125,CA153 and CEA. It can be used as an index to judge the curative effect and follow-up of breast cancer patients, and has centain clinical application value.
