摘 要:目的系统评价ERCC-2 rs1799793、rs13181位点单核苷酸多态性与胃癌患者预后相关性,为胃癌患者的预后评估及风险因子筛查提供新途径。方法检索Pubmed、Embase、Web of Science数据库,筛选2017年9月8日之前公开发表的胃癌与ERCC-2基因多态性相关的文献,应用STATA14.0软件进行进行meta分析,采用NOS量表对文献质量进行评价。最后进行发表偏倚评估及敏感性分析。结果共11篇文献、3282例经病理确诊的胃癌患者纳入研究,入选文献质量均为6分以上,rs13181位点单核苷酸多态性与胃癌患者预后相关性[GT/TT(HR=0.94,95%CI=0.80~1.10,P=0.408),GG/TT(HR=0.97,95%CI=0.75~1.25,P=0.820)],rs1799793单核苷酸多态性与胃癌患者预后相关性[AA/GG(HR=1.58,95%CI=1.20~2.08,P=0.001),GA/GG(HR=1.09,95%CI=0.93~1.27,P=0.280)]。结论 rs13181位点单核苷酸多态性与胃癌患者总体生存率无关,rs1799793位点的突变纯合子相对于野生型纯合子预后较差,总体生存率低。ERCC-2基因rs1799793位点的突变检测或可成为胃癌患者的预后评估及风险因子筛查的新途径。Objective To systematically assess the association between ERCC-2 single nucleotide polymorphisms(SNPs)and the prognosis of gastric cancer patients so as to develop a new approach for risk factors screening and the prognosis of gastric cancer patients.Methods Relevant studies were firstly retrieved from databases such as Pubmed,Embase,and Web of Science and screened the works(published before Sep 8 th,2017) that studied the association between the gastric cancer and ERCC-2 polymorphisms.Meta-analysis was performed by STATA(version 14.0) software and the NOS scale was used to assess the quality of the literatures.Finally,assessment of publication bias and sensitivity analysis were conducted.Results A total of 11 literatures and 3282 pathologically confirmed gastric cancer cases were included in this study.The scores of eligible studies are more than 6.The meta-analysis indicated the association between SNPs in rs13181 locus andprognosis of gastric cancer patients [GT/TT(HR=0.94,95% CI=0.80~1.10,P=0.408),GG/TT(HR=0.97,95% CI=0.75~1.25,P=0.820) ]and the association between SNPs in rs1799793 locus and prognosis of gastric cancer patients[AA/GG(HR=1.58,95% CI=1.20~2.08,P=0.001),GA/GG(HR=1.09,95% CI=0.93~1.27,P=0.280) ].Conclusions No significant association can be determined between SNPs in rs13181 locus and prognosis of gastric cancer patients.Patients with mutated homozygote AA in rs1799793 locus will have a poorer prognosis than the patients with wild genotype GG.Genotyping in ERCC-2 gene rs1799793 locus may become a new approach for prognosis prediction and risk factors screening in the treatment of gastric cancer patients.
