益气清热活血方治疗慢性萎缩性胃炎及其癌前期病变临床研究

摘　　要：目的:研究益气清热活血方治疗慢性萎缩性胃炎及其癌前期病变的临床疗效。方法:选择160例慢性萎缩性胃炎及其癌前期病变患者,将其随机分为对照组和观察组,每组80例患者,对照组采用胃炎宁颗粒治疗,观察组采用益气清热活血方治疗,连续治疗3个疗程后,观察对比两组患者临床治疗效果。结果:观察组治疗总有效率92.5%明显高于对照组72.5%,差异有统计学意义(P<0.05);治疗后两组临床症状评分明显降低,与治疗前对比(P<0.05),观察组降低明显低于对照组(P<0.05);治疗后两组萎缩、肠化、异型增生积分明显降低,与治疗前对比(P<0.05),观察组降低明显低于对照组(P<0.05);治疗后两组胃黏膜EGF,EGFR蛋白表达量、胃黏膜转化生长因子α(TGF-α)明显下降,与治疗前对比(P<0.05),观察组下降明显优于对照组(P<0.05);观察组HP根除率明显高于对照组,差异有统计学意义(P<0.05);两组患者均未发生不良反应,两组对比差异无统计学意义(P>0.05)。结论:益气清热活血方治疗慢性萎缩性胃炎及其癌前期病变的临床疗效显著,可显著减轻患者临床症状,抑制胃黏膜EGF,EGFR蛋白表达量,减轻病变组织的增殖活性,促进胃黏膜的修复,值得临床应用。Objective ：To study the clinical effect of Yiqi Qingre Huoxue decoction in treating chronic atrophic gastritis and precancerous lesions. Methods： 160 cases of patients with chronic atrophic gastritis and precancerous lesions were randomly divided into control group and observation group,80 cases in each group,The control group using Weiyanning granules treatment,observation group were treated with Yiqi Qingre Huoxue decoction treatment,continuous treatment of 3 courses （1 cycles of 1 months） after the comparison of two groups of patients with clinical curative effect. Results：The total efficiency of the observation group 92.5% was significantly higher than the control group 72.5%,the difference was statistically significant （ P 〈0.05）; the 2 groups after treatment of clinical symptoms scores decreased significantly,compared with before treatment （ P 〈0.05）,the observation group was significantly lower than the control group （ P 〈0.05）; the 2 groups after treatment atrophy,intestinal metaplasia,dysplasia score decreased significantly,compared with before treatment （ P 〈0.05）,The observation group was significantly lower than the control group （ P 〈0.05）;EGF of the 2 groups after treatment of gastric mucosa,the expression of EGFR protein in gastric mucosa,transforming growth factor A （TGF-a） were significantly decreased,compared with before treatment （ P 〈0.05）,the observation group were lower than that of control group （ P 〈0.05）; the observation group HP eradication rate was significantly higher than the control group,the difference was statistically significant （ P 〈0.05）;two groups of patients had no adverse reaction,no statistically significant differences between the two groups （ P 〉0.05）. Conclusion：The clinical curative effect of Yiqi Qingre Huoxue decoction in treating chronic atrophic gastritis and precancerous lesions significantly,can significantly relieve the clinical symptoms of patients with gastric mucosa,inhibit the expression of
