术前PET-CT p-SUV值与胃癌淋巴结转移相关性研究

摘　　要：目的通过探讨正电子发射计算机断层显像(PET)-CT标准化峰值摄取值(p-SUV)与胃癌淋巴结转移的相关性,评价PET-CT p-SUV值可否成为胃癌淋巴结转移独立预测因子。方法回顾性分析2014年3月至2018年3月解放军总医院收治入院并行D2根治性手术的胃癌病人,术前PET-CT及术后病理诊断报告保存完整,采用独立样本t检验,卡方检验、多元Logistic回归分析,分析胃癌淋巴结转移与PET-CT p-SUV值相关性。结果单因素分析中,肿瘤侵犯深度(P=0.035)和p-SUV值(P=0.000)与胃癌淋巴结转移具有相关性,在多因素相关性分析中,p-SUV值与胃癌淋巴结转移具有相关性,P=0.037。结论 PET-CT p-SUV值可成为胃癌淋巴结转移独立预测因子。Objective To study the PET-CT p-SUV values as the independent predictive factor for lymph node metastasis of gastric cancer by assessing the correlation between PET-CT p-SUV values and the lymph node metastasis of gastric cancer.Methods From March 2014 to March 2018,we analyzed the PET-CT of 71 patients that underwent D2 gastrectomy for gastric cancer according to the AJCC gastric cancer guideline.Statistical analysis was performed to determine the correlations between clinicopathologic parameters including p-SUV and lymph node metastasis using the chi-square test,the independent t-test,and logistic regression analysis.Results Univariate analysisp revealed that tumor invasive depth and primary tumor p-SUV were associated with lymph node metastases（P=0.035 and P=0.000,respectively）.The multivariate analysis showed that the p-SUV of primary tumor was independently associated with lymph node metastases（P=0.037）.Conclusions The p-SUV of primary tumor could be an independent predicative factor of lymph node metastasis in gastric cancer.
